Morbus Fabry-Anderson
A missing enzyme lets deposits grow silently — burning children’s feet are often the first sign, and it has been treatable for over twenty years.
Also known as: Fabry-Anderson disease · alpha-galactosidase A deficiency · GLA gene variant · acroparaesthesias · lysosomal storage disorder

A rare inherited metabolic disease in which one enzyme is missing or too weak — alpha-galactosidase A. As a result a fatty substance gradually builds up in vessels, nerves, kidneys and heart. The first and often for decades only sign is burning pain and abnormal sensations in hands and feet, so-called acroparaesthesias: about 80 percent of those affected know them already between the ages of five and ten. Attacks last minutes to days and are typically triggered by fever, infection, exertion, heat, tiredness or stress. In adulthood they often ease — not because things improve, but because nerve fibres are lost. Further parts of the picture: reduced sweating with very dry skin and poor tolerance of both heat and cold, reddish-purple skin spots mainly in the swimwear area, abdominal pain and diarrhoea, and a fine, wheel-spoke-like corneal clouding that nearly everyone affected has and that an eye doctor sees at the slit lamp — it does not disturb vision but is a valuable clue. Later come protein loss through the kidneys from about age 20, thickening of the left heart chamber in around a third, and an increased stroke risk, in some before the age of 50. Inheritance is X-linked; for a long time only men were considered affected, today it is clear that women can fall ill too — sometimes markedly. Affected fathers pass the trait to all daughters, affected mothers with 50 percent to all children. The decisive point: Fabry disease is treatable. Since 2001 there has been enzyme replacement therapy as an infusion every two weeks, and depending on the gene variant an additional oral drug. The earlier the diagnosis, the better the long-term course — which is why Fabry is worth thinking of with unexplained burning pain in hands and feet since childhood.
Fever, infections, heat, exertion, tiredness and stress typically trigger the burning pain. Knowing your patterns allows prevention rather than only reaction.
Because sweating is reduced, natural cooling is missing. Shade, cooling, drinking, cooler times of day for exertion — this is not over-caution but sensible adaptation.
Protein loss in the urine often begins silently from about age 20. Regular checks catch it early, when there is most to preserve.
Around a third develop cardiac involvement, about a quarter cerebral perfusion problems. Cardiological and neurological follow-up are therefore part of care.
The wheel-spoke corneal clouding does not disturb vision but is present in nearly all and very helpful for diagnosis — seen at the slit lamp.
There is enzyme replacement therapy and, depending on the gene variant, an oral drug. Timing matters: the earlier treatment starts, the more organ function is preserved.
An affected father passes the trait to all daughters, an affected mother with 50 percent to all children. That women can fall ill too was long underestimated — genetic counselling puts this in context calmly.
The attacks are real and often severe — those affected were long told it was “growing pains” or psychological. Good pain management is part of treatment.
Enzyme replacement supplies by infusion every two weeks what the body does not make — it slows the build-up, it does not clear damage already done to kidney and heart. So the timing of the start matters more than the drug itself. Migalastat as a tablet works only with certain gene changes, because it stabilises the person’s own misfolded enzyme. Against the burning pain in hands and feet, nerve pain drugs work, not classic painkillers.
About one in forty to sixty thousand men is affected; women carry the change too and are often affected later and more mildly — contrary to what was long assumed, they are not merely carriers. The burning pain in hands and feet usually begins in childhood, between five and fifteen. The time to diagnosis averages twelve to fifteen years. Kidney and heart involvement appear from the third or fourth decade.
From childhood or youth, in attacks, set off by fever, heat, sport or stress. They are often taken for growing pains or rheumatism — and that for decades.
Anyone who hardly sweats during sport or in the sun overheats quickly and avoids warmth. This combination together with the burning pain is very typical.
Small, flat to slightly raised red vessel dots in the swimming-trunks area, on buttocks and thighs. They do not hurt and are therefore hardly ever mentioned.
Protein in the urine, a thickening of the heart wall, rhythm disturbances, hearing loss and ear noises, belly pain with diarrhoea. When several organs come together without fitting, that is the occasion for the question.
The kidneys are among the main organs affected. A slow decline over years is an important sign of progression.
Rises only once kidney function has already clearly declined — judge together with eGFR.
A pointer to cardiac strain; in Fabry, thickening of the left heart chamber is typical.
Can be mildly elevated with cardiac involvement — interpretation belongs with cardiology.
They lower the blood fats and act on the excitability of the heart muscle. Oily sea fish twice a week is the best-supported route; very high doses from capsules are not a harmless more.
Without B12 the insulating layer of the nerves is not maintained. A lack causes tingling, numbness and unsteadiness in the dark — and it is one of the few reasons for nerve trouble that can be fully undone if found early.
Keeps the rhythm steady and lowers blood pressure. Water tablets drive it down, certain blood pressure drugs and a weak kidney drive it up — which is why it is checked under treatment.
The nerves cover their energy needs through thiamine. It runs short with alcohol, after stomach operations, with long-lasting vomiting and under water tablets.
A lack favours extra beats and at the same time keeps a potassium deficiency going. Anyone replacing only the potassium sees no success while the magnesium is missing.
Often used in storage diseases. No benefit is shown for Fabry disease — enzyme replacement therapy is the route that works, and it belongs with a specialised centre.
Effects on inflammation and triglycerides established; cardiovascular prevention is mixed. Watch quality and freshness.
Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.
Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.
In children the condition often begins with burning pains in hands and feet with heat and fever — and with an absence of sweating. Delayed puberty and lagging growth come along in boys.
The kidney loses the ability to concentrate the urine early — large amounts of urine and thirst are among the first signs, long before the kidney values change.
Belly pain and diarrhoea after eating belong to the picture. Smaller, lower-fat portions are tolerated better by many; a food diary over 2–4 weeks shows the personal pattern.
In Fabry disease heat and exertion often trigger pain attacks. Movement is still worthwhile — cool, steady and in smaller units rather than intense.
