The pain is real, the volume is the problem — and the most effective first step is movement that deliberately begins too small.

In fibromyalgia it is not the tissue that is ill but the processing of pain. One can picture it as a volume turned up too far: stimuli that normally do not hurt arrive in the nervous system as pain. That does not make the pain any less real — it only explains why nothing is to be found in X-rays and blood values. Typical are widespread pains over more than three months in several regions of the body, along with an exhaustion that stays even after sleep, sleep without refreshment, and difficulty with concentration and memory. The diagnosis is made positively, from this pattern — it is not what remains once everything else has been excluded. A small set of laboratory tests is enough to recognise the main look-alikes; ever more investigations by contrast do more harm than good, because they keep alive the search for tissue damage that is not there. What helps is well studied and surprising in its order: first comes movement increased step by step, then sleep, then understanding the mechanics behind it. The way in matters — start too small and increase slowly, otherwise the setback follows.
Five to ten minutes of brisk walking or movement in warm water, every other day, and only after two weeks a little more. It is the measure with the best record — it fails almost only on too large a start.
A fixed rising time, a cool dark room, no caffeine after midday. Better sleep measurably lowers the strength of the pain — it is not a side issue.
Thyroid, inflammatory markers, blood count, calcium, CK and vitamin D are enough to recognise the look-alikes. After that, searching further no longer helps but keeps a false expectation alive.
In water the buoyancy carries part of the body weight, and the warmth lowers muscle tension. For many it is the only form of movement that works at the start without a setback.
Lack of sleep, fear of movement, lasting stress and setbacks all turn the same screw. Anyone who understands the mechanism gets out of the circle of resting and worsening.
They achieve little with this kind of pain, cause dependence and can even raise pain sensitivity over time. This piece of information saves a long detour.
About two in a hundred people are affected, women markedly more often. Several years and many investigations pass on average before the diagnosis — because all findings are normal and the condition cannot be imaged.
Classic painkillers and anti-inflammatories do little here, because the pain does not come from inflamed tissue but from processing. What works in studies acts on that processing: low-dose amitriptyline in the evening, duloxetine, pregabalin — they need four to eight weeks and halve the pain in about a third of people. Endurance movement in small doses, raised slowly over three months, stands level with them in the reviews.
The pain sits in several regions of the body at once, wanders and changes in strength. A pain that always stays in exactly one place does not fit the picture.
One sleeps but wakes up wrecked. Sleep is the lever that changes the pain most — and the other way round.
Word-finding, concentration, short-term memory. It belongs to the illness and is not a sign of something additional.
Doing too much on a good day is paid for over days. That is exactly why movement is deliberately begun too small and increased in small steps.
These companions often occur together, because the same over-sensitivity underlies them. Treating them often improves the pain as well.
Widely spread rather than in one spot, over months, with sleep that does not restore. The muscles themselves are unremarkable — that is part of the picture.
An underactive thyroid causes muscle pain, exhaustion and trouble concentrating — the most important look-alike, and separated by a single value.
In fibromyalgia they are normal. If they are raised, an inflammatory disease is in the room and the direction changes.
The value for the muscle itself. It separates muscle pain from genuine muscle damage — under cholesterol-lowering drugs for instance — from altered pain processing.
A marked shortage can cause muscle and bone pain of its own. It is simple to correct — but it is not the explanation for the whole picture.
Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.
Both are more often low in fibromyalgia. Studies on correction show small effects on pain and sleep; measure first, supplement after.
Gluten-free, low FODMAP and others are recommended; the studies are small and inconsistent. Anyone trying should test for 4 weeks and then judge honestly rather than restrict permanently.
Gentle, measured movement is the best-documented measure — and the hardest.
Does not change the pain itself but the way it is lived with — measurable in studies.
