Losing weight without intending to is always a reason to look — even if it feels good at first.
Also known as: unintentional weight loss

When weight drops noticeably without intending it (rule of thumb: more than about 5 % in six months), a medical check is worthwhile — not out of fear, but to calmly find the cause. Common and treatable reasons are an overactive thyroid, unrecognised diabetes, mental strain or depression, absorption problems (e.g. coeliac disease) or ongoing inflammation. Less often more serious illnesses are behind it — which is exactly why the check makes sense, and the result is usually reassuring. The key is to tell a true unintended loss from the normal ups and downs of weight.
A weight loss that arises because someone eats less from experience looks like loss of appetite — but it is something else. The question is: does it hurt after eating, and does the pain grow with the portion?
An unintended loss of more than five per cent of body weight in three to six months is the threshold at which it gets a name and an approach of its own — even at a normal weight.
A clear, unintended weight loss should be assessed; usually with a reassuring result.
Thyroid, blood sugar and inflammation markers clarify many causes.
Appetite, digestion, fever or night sweats help narrow the direction.
Until the cause is clear, a sufficient, nourishing diet helps.
In children and after travel: giardia slows absorption in the small bowel while the blood count stays normal.
Raised eosinophils with weight loss or abdominal complaints after a journey are a lead worth following — even if the stool was normal.
Beef and pork tapeworm often cause nothing but changing appetite and slow weight loss. They usually announce themselves through moving white segments in the underwear.
Single-celled organisms such as cryptosporidia and cyclospora cause diarrhoea that lasts weeks and costs weight. With weakened defences it does not stop without treatment. A threadworm silent for decades wears the body down in the same way.
An unintended loss of more than five per cent of body weight over six to twelve months warrants a work-up — four kilos at eighty. In about a quarter no cause is found despite a full work-up. The most common graspable causes are cancer, gut disease, thyroid, diabetes, medicines and depression, and that is usually the order in which they are looked for.
There is no drug for unintended weight loss — what is prescribed targets the cause, and finding it is the actual step. Appetite stimulants work weakly and bring mainly fat and water; megestrol raises the risk of thrombosis. Nutritional drinks supply calories and are given between meals, because otherwise they take the appetite away. With severe undernutrition, the first week of refeeding is the critical one.
From about this magnitude, the cause should be checked.
Two common, treatable causes that are easy to test.
Stress, worries or depression can reduce appetite and weight.
Digestive diseases like coeliac mean food is less well absorbed.
The single most important hint of worms in the blood. They are raised with worms that travel through tissue — not with giardia, threadworms and amoebae, which stay in the bowel. A normal value therefore rules nothing out.
The loss of muscle mass helps decide how well a treatment is tolerated. Enough protein spread across the day, together with movement, is the part one holds oneself.
Low values are common with a longer illness, and correcting them counts for bone and muscle. For high-dose courses against the illness itself there are no studies; they can interfere with treatment.
Belong in the workup of an anaemia that is otherwise too quickly put down to the illness or the treatment. Both are easy to measure and easy to supplement.
Nausea, diarrhoea and fever dry one out quickly, and several treatments strain the kidney. Drinking enough is no side issue here.
Fills gaps but explains nothing. Losing more than 5 per cent of body weight unintentionally in 6 months needs a cause — and a capsule only pushes the search back.
Where appetite is missing and muscle mass is shrinking, a shake brings protein in a small volume — 20–30 g a portion. That holds muscle, but does not replace the workup.
Clearly worthwhile and well studied when deficient; fewer data for the wider uses. Fat-soluble — check blood levels at high doses.
Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.
Losing weight with a good or increased appetite is the mark of an overactive thyroid. This axis belongs checked with every unintended loss.
A newly appeared diabetes causes weight loss, because sugar is lost through the urine. Together with thirst and passing much water this is an urgent hint.
A shortage causes weight loss with loss of appetite, nausea, darkened skin and low blood pressure. This combination is rare, but it belongs recognised.
With lasting inflammation or a tumour illness the hunger signal falls while the body uses more at the same time. That explains the loss despite eating normally.
More than 5 % of body weight in 6 months without intending it warrants investigation — at 70 kg that is 3.5 kg. Not a diet question but a reason to look.
Anyone with little appetite cannot manage large portions. Oil, nuts, full-fat dairy and avocado bring a lot of energy in a small volume — 100 g of nuts is about 600 kcal.
Energy density rather than volume: oil, nuts, full-fat dairy. But the cause belongs looked for first — more than 5 % in 6 months without intending it is a reason to investigate, not a nutrition topic.
