Torsio testis
A time emergency: sudden severe testicular pain belongs in the emergency department at once — hours count.
Also known as: testicular torsion · twisted testicle · acute testicular pain · twisted spermatic cord

The testicle hangs from the spermatic cord, and its blood vessels run inside that cord. If the testicle turns on its own axis, they are pinched off — the testicle gets no blood. This hurts suddenly and severely, often out of sleep or after movement, usually on one side, frequently with nausea or vomiting; the scrotum is red, swollen and tender, and the affected testicle often sits higher and crosswise. Mainly adolescents and young men are affected, but newborns too. What matters here is not the diagnosis but the clock: if the testicle is untwisted within about six hours it is as a rule preserved; after that the outlook drops with every hour. So one sentence applies without qualification — sudden severe testicular pain belongs in the emergency department at once, not in tomorrow’s appointment and not in waiting it out with an ice pack. Even if the pain eases in between: a twist can undo itself and come back. Testicles have been lost because someone waited out of embarrassment. That is the only reason this entry is here. The guideline sets the mark at four to six hours for surgery; beyond eight hours the share of testicles lost despite the operation rises markedly. And one more thing belongs here: anyone who has had a sudden testicular pain that went away on its own should mention it. A twist that undoes itself and returns is a finding in its own right — there is a small preventive operation for it that heads off the emergency.
Do not call and wait, not until tomorrow. Within about six hours the testicle is usually preserved.
If an operation becomes necessary, an empty stomach is worth it.
The most common reason for arriving too late is not ignorance but embarrassment.
With boys, what counts is that someone looks, asks and shortens the way.
The window decides: within six hours the testis is saved in about nine out of ten cases, after twelve hours in about half, after twenty-four hardly at all. Newborns and boys between twelve and eighteen are most often affected. The pain begins suddenly, often at night, with nausea — and the testis sits higher than usual. With suspicion, surgery follows without waiting for an ultrasound; the other testis is fixed during the same operation.
No drug undoes a torsion. Painkillers must not delay things — here only the time to surgery counts. Even a normal ultrasound does not rule it out reliably; with a typical picture, surgery follows and the surgeon looks. During the operation the other testis is fixed too, because the predisposition is on both sides. If surgery comes too late, no drug helps — the testis is lost, fertility usually stays.
Out of nowhere, often at night or after movement — not a slow worsening.
A frequent companion and a sign of how strong the stimulus is.
Touch is barely bearable; the testicle often sits higher than usual.
A twist can undo itself and return — that is not an all-clear.
The most common age group — it also occurs in newborns.
Sudden, very severe, often at night or out of sleep, usually one-sided and with nausea. Here time counts — the blood supply is cut off.
With sudden testicular pain the immediate examination is what counts. CRP is usually raised in epididymitis and normal in torsion — no value may delay treatment.
Skin and lining renew themselves quickly and need zinc for it. With places that heal badly, cracked corners of the mouth and frequent skin infections a lack is a serious possibility.
Without vitamin C no stable collagen forms — the wound closes but holds less well. With a very one-sided diet, smoking and alcohol the intake is often tighter than assumed.
Steers the horn-forming of the skin. Too little makes it rough and dry, too much over longer periods harms liver and bone — and in pregnancy a strict upper limit applies.
Every healing needs building material. Anyone eating little during an illness loses exactly that — a sufficient amount spread across the day belongs to the treatment.
The decisive point is the clock: within six hours the testicle can usually be saved, after that the outlook falls steeply. Sudden severe testicular pain is not something to wait out.
Even after a successful operation sperm production can suffer — on both sides, not only the affected one. With a wish for children a semen analysis is worth it, however well it looks healed.
Only one thing stands here: with sudden severe testicular pain 4–6 hours decide. Cooling, waiting or a home remedy cost the testicle in that time — the route leads straight to the emergency department.
