The bone shape alone is not a diagnosis — many carry it without ever having complaints.
Also known as: hip impingement · FAI · groin pain · C sign · labrum · hip pain when sitting

A striking of the femoral neck against the socket rim when the hip is bent and turned inwards. The pain typically sits in the groin, and those affected often show it with thumb and index finger as a C around the hip — that sign is so characteristic that it has a name of its own. Complaints arise with deep sitting, getting into a car, pulling on socks and in sports with deep movements. Two forms of bone shape underlie it: too broad a transition at the femoral neck, too deep or too far-covering a socket, often both together. Important for placing it is a number from imaging studies: many people carry the same bone shape without ever getting complaints — the shape alone is therefore not a diagnosis, only together with matching complaints and a matching examination.
Showing the pain as a C with thumb and index finger around the hip almost always means the joint itself. Tapping flat on the outside points to the bursa or the tendon attachments — two quite different answers.
Deep, soft sitting with the knees above the hip is the position in which it strikes. Sit higher, knees below hip height, and when getting into a car sit down first and then draw the legs in — that takes out the everyday share.
In comparisons with an operation a structured exercise programme brought similar improvements in some of those affected — and it is the route that closes no doors. Twelve weeks is the frame in which that can be judged.
A tear of the joint lip is also found in many people without any complaints. It explains the pain only when place, triggering movement and examination fit together — otherwise it is an incidental finding like grey hair.
The impingement arises where head and socket meet. No deficiency sign leads to this shape — it usually formed during growth.
The shape of the femoral head and socket is judged on X-ray, the cartilage and labrum on MRI. Blood values only come in to rule out something else, if the joint is warm, swollen or painful at night.
To rule out. Impingement is a question of shape and does not raise inflammatory markers. A warm, swollen hip with raised CRP is something else and should be looked at promptly.
The counterpart of calcium at the muscle fibre: calcium contracts, magnesium releases. With tension and night cramps it is therefore the first thing asked about — the studies are mixed, the mechanism at the muscle is not.
When walking, several times the body weight acts on the hip. With a change in shape this decides how often the tight spot actually catches during the day.
The best-selling joint preparations and the best studied. The large GAIT trial found no advantage over dummy treatment across two years. In impingement it is added that this is a problem of bone shape and not a lack of cartilage — from a cartilage building block nothing is to be expected there anyway.
The route via strength: a low level weakens the gluteal muscles, and it is precisely they that hold the head of the femur centred in the socket. Measured and replaced, that is a precondition for the exercise programme — not its replacement.
Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.
The growth spurt is when something shifts in the spine — and the time when it can still be influenced. After the growth plates close, the shape is largely set.

It is not the session that works but what continues at home afterwards. After four to six appointments you can judge whether anything is moving.
For everything to do with statics, the method with the most data. What matters is not the kind but that it happens at all, and regularly — the back holds what it does daily.
