The point can be shown with one finger, and the pain ends above the knee — that is the difference from sciatica.

A pain at the side below the low back, often on one side only, with a point that can be well shown just beside the midline. It sometimes radiates into the buttock or the back of the thigh but typically ends above the knee — that distinguishes it from sciatic pain, which travels further. The joint itself moves only a few millimetres and is secured by very strong ligaments; a “blockage” in the literal sense can therefore hardly be spoken of, and the widespread manoeuvres for detecting it are unreliable taken singly. What has proved usable is the combination of several provocation tests: if three out of five respond, the joint is probable as the source. It becomes more common in pregnancy and after birth, because the ligaments are softer, and after a fall onto the buttocks.

Levers & pathways

studiedOne finger rather than a hand

Pointing to the pain with one finger on a spot beside the sacrum points to this joint. Stroking the whole hand over the low back usually means something else. This small distinction often saves a detour.

studiedWhy it becomes more common in pregnancy

The ligaments become softer so that the pelvis can widen for birth — and softer ligaments means more play where there is otherwise hardly any. That is not a malfunction but the purpose. What helps is strength in glutes and pelvic floor and a pelvic belt if needed, not rest.

studiedStrength beats manipulation

Manipulations can relieve in the short term, and the crack in the process is a gas bubble in the joint and not something springing back. What changes the course is stabilisation through gluteal and trunk muscles over weeks — the part that manages without an appointment.

studiedWhen it is something else

Bilateral, deep-seated pain with morning stiffness over half an hour that improves with movement and worsens with rest, particularly in younger people, belongs investigated — that is the pattern of an inflammatory condition and not of a mechanical irritation.

Possible complaints

studiedVitamin D

Low values over a long time cause a dull ache in bones and trunk-near muscles, morning stiffness and weakness when rising from a chair. This is often blamed on posture.

Blood values

studiedCRP

The one question this value answers: inflammatory or mechanical. A blocked sacroiliac joint does not raise CRP. If it is raised and the pain wakes you at night and eases with movement rather than rest, the direction changes.

studiedESR

The slower sibling of CRP. Together the two say more than either alone, because they react at different speeds.

infoHLA-B27 only with a reason

This genetic marker test is used when an inflammatory spine disease is suspected. It is not a screening test: many people carry the marker and never fall ill, and some of those affected do not carry it. It answers a question already asked; it does not ask it.

Nutrients

mixedMagnesium

The counterpart of calcium at the muscle fibre: calcium contracts, magnesium releases. With tension and night cramps it is therefore the first thing asked about — the studies are mixed, the mechanism at the muscle is not.

mixedVitamin D

Belongs to telling things apart: low values over a long time cause a dull, deep low-back pain that resembles a blockage and does not release with movement.

Supplements

mixedVitamin D3 (+K2)

Not as a treatment but for the distinction: a marked vitamin D deficiency can make bone pain in the pelvis that resembles a joint irritation, and unlike it, it is correctable. With bilateral, dull pelvic pain it belongs measured alongside.

studiedWhy nothing else stands here

The joint moves a few millimetres and is secured by very strong ligaments. What changes the course is stabilisation through gluteal and trunk muscles over weeks. No preparation intervenes in that mechanics — and an agent recommended against it promises more than there is to be had here.

infoMagnesium (glycinate/citrate)

Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.

Hormones

associatedRelaxin and estradiol

During pregnancy and for some months after, the sacroiliac joint is more mobile than usual. Complaints there are the rule in that period, not the exception.

Food

infoWhat indirectly unloads the back

The plate does not act on the joint itself. What acts is everything that keeps the abdomen quiet: regular meals and enough fibre — hard straining loads the pelvis from within.

Alternative approaches — what people use

mixedOsteopathy & chiropractic

Can quickly release a limited range of motion. What it cannot do is change the cause that created it — if the restriction keeps returning, the reason lies elsewhere. At the neck, warning signs are asked about first.

studiedMovement & exercise

For everything to do with statics, the method with the most data. What matters is not the kind but that it happens at all, and regularly — the back holds what it does daily.

studiedPhysiotherapy

It is not the session that works but what continues at home afterwards. After four to six appointments you can judge whether anything is moving.
