Forward head posture (“text neck”)

DE FR EN

The calculation is right, the conclusion from it is not — what tires is the holding muscles, not the bone.

Also known as: forward head posture · text neck · head forward · phone neck · neck load

The head does not stand over the spine but in front of it. The number behind it is memorable: a head weighs four to five kilograms, and for every centimetre it travels forward the load on the neck muscles rises by about a kilogram; at the tilt taken up when looking at a phone, the calculation gives several times the weight of the head. That calculation is right as mechanics — what it does not prove is the conclusion that damage inevitably follows. In long-term studies the extent of forward head carriage did not reliably predict later neck pain, and many people with a marked forward head have no complaints. What does show: the deep neck muscles that hold against it tire — and with them, towards the afternoon, comes the tension that makes the typical headache.

Levers & pathways

studiedRaise the screen rather than lower the head

The top edge of the screen at eye level. On the phone: lift the device rather than lower the head — the tilt of the head is the quantity the load hangs on, and it is the only one that can be changed at once.

studiedThe double chin as an exercise

Push the head horizontally backwards as if making a double chin, without nodding. Hold ten seconds, several times through the day. That reaches exactly the deep neck muscles at issue — and it looks silly, which is why it is mostly practised in the car or alone.

studiedThe thoracic spine first

A rounded upper back inevitably pushes the head forward — the neck sits on top of it. Working only at the neck and leaving the thoracic spine stiff fights against the foundation.

studiedThe pillow — the one number that counts

Not the brand but the height: it should hold the neck in line with the spine, in the position one actually sleeps in. Side sleepers need more height than back sleepers; whoever sleeps face down turns the neck all night.

studiedThe eyes lead, the head follows

At the screen the gaze stays fixed on one point, and the neck holds still with it. Leading with the eyes when looking away and letting the head follow goes further and with less tension. The short muscles under the occiput are decisive here — they report head position to the brain and respond better to small, frequent movements than to long stretches.

studiedRaise the device instead of lowering the head

The further the head hangs forward, the greater the weight the neck holds. Every ten minutes look up and gently draw the chin back.

Possible complaints

studiedVitamin B12

Tingling in hands and feet, unsteady walking in the dark, forgetfulness. Anyone eating purely plant-based or taking metformin or acid blockers has this topic regardless of posture.

mixedMagnesium

Night calf cramps, twitching eyelid, a neck that will not release. The signs are unspecific — they work as a question, not as proof.

Blood values

infoNot a lab value but a look from the side

What is measured is the distance from the ear canal to the plumb line through the shoulder. The rest is observation: how long at a stretch, at what height, with what breaks. Blood values only come in if numbness or loss of strength reaches the arm.

mixedVitamin B12

Only with tingling and numbness in hands and arms: a B12 deficiency causes the same complaints as a compressed nerve in the neck — and is therefore often blamed on posture.

Nutrients

mixedMagnesium

The counterpart of calcium at the muscle fibre: calcium contracts, magnesium releases. With tension and night cramps it is therefore the first thing asked about — the studies are mixed, the mechanism at the muscle is not.

studiedPotassium

Belongs to the same question as magnesium. Sweating a lot, eating little or taking diuretics loses both together.

mixedVitamin B12

Only if tingling or numbness comes with it. A deficiency causes the same sensations as a compressed nerve in the neck — and is then blamed on posture, though it has nothing to do with it.

Supplements

mixedMagnesium

Frequently taken when the neck is hard in the evening. Where it helps, it helps with a proven deficiency and via sleep. On the position of the head it does not act — that hangs on the endurance of the deep neck muscles and on the height of the screen.

infoCaffeine in the afternoon

The tension in the neck typically comes in the late afternoon, when the holding muscles are tired. Caffeine after that worsens deep sleep; in shallower sleep clenching and tensing happen more often — and the next afternoon it starts earlier. Two weeks without coffee after midday answer the question on oneself.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

Hormones

associatedCortisol

The neck is where tension settles first. Forward head posture arises at the screen — but it stays even when the screen is off and the tension is not.

Food

infoThe plate is a side issue here

Forward head posture comes from screen time and screen height, not from food. Only if tingling or numbness comes with it is B12 worth a look — especially on a purely plant-based diet.

Alternative approaches — what people use

studiedMovement & exercise

For everything to do with statics, the method with the most data. What matters is not the kind but that it happens at all, and regularly — the back holds what it does daily.

studiedPhysiotherapy

It is not the session that works but what continues at home afterwards. After four to six appointments you can judge whether anything is moving.

mixedMassage & manual therapy

Releases tension for hours to days and feels good in that time. On its own it changes nothing about posture, because it does not strengthen the muscles.

traditionalAtlas correction

Work on the short musculature under the occiput, usually in one session and without a thrust on the joint. No human studies exist; head rotation before and after is measurable.
