Mast cell activation syndrome (MCAS)

DE FR EN

A diagnosis with real symptoms whose boundaries are still developing — telling it apart needs care rather than quick labels.

Also known as: MCAS · mast cell activation · mast cell disorder

This compilation is for information and replaces neither medical diagnosis nor treatment. If you have symptoms, see a doctor.

In MCAS, mast cells — immune defence cells — react too easily and too often and release their messengers (e.g. histamine). This triggers allergy-like symptoms in several organs at once: skin (flushing, hives, itch), gut, circulation and breathing, often from many different triggers. The boundaries are still developing: clear criteria exist (matching symptoms + a measurable rise in mast-cell markers during an episode + response to treatment), and MCAS is often diagnosed too readily. So a careful work-up matters. To be distinguished from the rare mastocytosis (too many mast cells).

Levers & pathways

studiedIdentify & avoid triggers

A symptom diary helps find and avoid personal triggers.

associatedAntihistamines & stabilisers

H1 and H2 antihistamines and mast-cell stabilisers ease symptoms — medically guided.

associatedCareful diagnosis

Because of over-diagnosis risk, have the criteria checked properly rather than labelling too quickly.

associatedDiscuss an emergency plan

For severe reactions, agree an emergency plan with the doctor.

infoWhat the medicine does — and what it does not

The drugs occupy the docking sites of the messengers or keep the mast cell quiet — antihistamines against H1 and H2, plus cromoglicate, which stays in the gut and needs four to six weeks before it can be judged. They cure nothing and they do not lower the number of mast cells. It is built up in steps over weeks, because otherwise it cannot be said which part worked.

infoNumbers and timeframes

Three things count together for the diagnosis: recurring complaints in at least two organ systems, a measurable rise in a mast cell messenger within four hours of the episode, and a response to drugs that act on mast cells. Tryptase is measured during the episode and again at rest after twenty-four hours — only the rise between the two counts. Building up the treatment takes four to eight weeks, because it is added in steps.

Possible complaints

associatedSkin: flushing, hives, itch

Sudden flushing and hives, often in episodes.

associatedGut symptoms

Abdominal pain, cramps, diarrhoea or nausea during reactions.

associatedCirculation & many triggers

Dizziness or blood-pressure drops; reactions to varied triggers (heat, stress, food, scent).

Blood values

studiedTryptase

The central value. What speaks is the comparison: once during an episode, shortly after it starts, and once in a quiet phase as a baseline.

associatedTotal IgE

Serves the distinction: an allergy is the commoner explanation for the same complaints and is looked for first.

associatedCRP

Usually stays normal here. A raised value directs the view to an inflammation or an infection as the real cause.

Nutrients

associatedVitamin D

With chronic inflammatory and autoimmune pictures low levels are common — partly because the illness itself lowers the level, partly because less time is spent outdoors. Whether correcting it changes the course is being studied; for bone and muscle it is worthwhile regardless.

associatedOmega-3 fatty acids

They give rise to messengers that tend to end inflammatory reactions rather than fuel them. Oily sea fish twice a week is the simplest route; capsules are an option but replace no treatment.

associatedZinc

Needed for steering the defences and for the healing of tissue. A lack is more common with chronic diarrhoea, little meat and pulses, and under cortisone.

associatedSelenium

Part of the body’s own protection against oxidative stress. The need is covered by Brazil nuts, fish, eggs and offal — more is not better here, and high doses over long periods are not harmless.

Supplements

mixedQuercetin

The most-named remedy in this field — 500 mg twice daily. The effect on mast cells is shown in the lab, barely in humans; uptake from the capsule is low, somewhat better with fat.

mixedVitamin C

Helps break histamine down through its own enzyme route; in small studies histamine levels fell at 1–2 g a day. Above 1000 mg a day long term it is unfavourable with a tendency to oxalate stones.

mixedDAO (diamine oxidase)

Taken before the meal to break histamine down in the gut. The studies are small, the preparations expensive — and it is a barrel, not a switch: what counts is what adds up over a few hours.

infoVitamin D3 (+K2)

Clearly worthwhile and well studied when deficient; fewer data for the wider uses. Fat-soluble — check blood levels at high doses.

infoOmega-3 (EPA/DHA)

Effects on inflammation and triglycerides established; cardiovascular prevention is mixed. Watch quality and freshness.

Hormones

studiedHistamine

The messenger everything turns on here: it widens vessels and causes wheals, palpitations, headache and diarrhoea. A single blood value says little — the timing in relation to the episode counts more.

associatedCortisol

The body’s own brake on over-shooting inflammatory reactions. That explains why episodes come more often in times of great strain.

associatedOestradiol

Mast cells carry docking sites for this hormone. So many people describe episodes tied to the cycle — a pattern that can be written down.

Food

studiedLow-histamine for a while

Aged cheese, red wine, smoked fish, sauerkraut and long-stored food are high in histamine. A trial of 2–4 weeks shows whether it applies; held strictly for longer the diet narrows.

studiedFreshness counts more than the food list

Histamine forms with storage time. The same fish is tolerated fresh and not after two days in the fridge — freezing right after purchase helps more than avoidance.

Alternative approaches — what people use

mixedVitamin C

Vitamin C takes part in breaking down histamine and is widely used in mast cell complaints. Controlled studies are largely lacking; this is experience, not proof.

studiedDietary change

A low-histamine phase of 2–4 weeks shows whether it applies. Then it is widened again — held strictly for good the diet narrows and the fear of eating grows.

TCM — how it is seen there

traditionalPattern diagnosis

A picture that appears in many organs at once and fits no single box — the pattern diagnosis is built for exactly that. It explains nothing about mast cells, but it describes what otherwise falls apart.
