The minutes decide: short and in large joints means wear, long and in both hands means inflammation.

Almost everyone is a little stiff in the morning. The decisive question is not whether but how long. Stiffness that disappears after a few minutes of movement and sits mainly in the large, loaded joints — knee, hip, base of the thumb — fits wear: cartilage needs movement to glide again, and the first steps hurt. Stiffness lasting longer than half an hour to an hour, together with swollen knuckle joints on both hands, is a different pattern and points to an inflammatory joint disease — here every week counts, because early treatment preserves a great deal. Stiffness deep in the back and pelvis in young adults, improving with movement and waking them at night, is a third route, often recognised late. And stiffness in the shoulder and hip girdles in people over fifty, with exhaustion, is a fourth. Writing down how many minutes it lasts and which joints are involved is already the most important part.
Stiffness low in the back that lasts half an hour or longer in the morning and only loosens with movement, together with waking at night, belongs to the inflammatory form — especially in younger people.
Morning stiffness of clearly more than half an hour, on both sides at shoulders and hips, with an almost datable onset: that is a picture of its own and not osteoarthritis.
For one week: how long until you move freely? No test replaces this number.
Symmetric and in small joints is something other than one knee alone.
A warm shower and a few calm movements before getting up shorten the start-up time.
Over an hour with swollen fingers is a reason to make an appointment soon.
Hot water bottle, cherry stone cushion or a warm bath, 20 to 30 minutes, several times a day — and move afterwards, do not stay lying down.
With back and joint pain keep moving within the pain-free range, keep everyday life going, avoid bed rest.
The duration is what distinguishes: over thirty to sixty minutes points to an inflammatory cause, under thirty rather to wear. In rheumatoid arthritis it often lasts over an hour, in osteoarthritis rarely more than half. In polymyalgia rheumatica it affects shoulders and hips and lasts hours. Noting the duration over two weeks brings the doctor the single most informative piece of information.
Anti-inflammatories work if there is inflammation behind it — and then usually clearly within days; with wear the difference is small. That response is itself information. In polymyalgia rheumatica, low-dose cortisone improves the stiffness within twenty-four to forty-eight hours — if it does not, the classification is usually wrong. With wear, what works is movement before getting up and warmth, not a drug.
Start-up pain with wear — movement improves it quickly.
With swollen knuckles, the pattern of an inflammatory disease.
In young adults, better with movement, worse at rest.
With exhaustion, a distinct and very treatable picture.
Slower than CRP and therefore usefully complementary, especially in older people.
One building block in deep back and pelvic pain of young adults — never conclusive alone.
With chronic inflammatory and autoimmune pictures low levels are common — partly because the illness itself lowers the level, partly because less time is spent outdoors. Whether correcting it changes the course is being studied; for bone and muscle it is worthwhile regardless.
They give rise to messengers that tend to end inflammatory reactions rather than fuel them. Oily sea fish twice a week is the simplest route; capsules are an option but replace no treatment.
The building material itself. Better from food than from a tin: hard cheese, yoghurt, calcium-rich mineral water, almonds, broccoli and fortified plant drinks.
Needed for steering the defences and for the healing of tissue. A lack is more common with chronic diarrhoea, little meat and pulses, and under cortisone.
Activates the proteins that build calcium into bone. Green leafy vegetables and fermented foods cover it — under coumarin blood thinning the amount stays steady rather than swinging.
With inflammatory joint rheumatism, morning stiffness is the point where studies most often measured something — 2–3 g EPA+DHA a day over at least 3 months. With wear-related osteoarthritis the same amounts show little.
500–1000 mg a day over 8–12 weeks; without piperine or a special formulation little arrives. With gallstones and on blood thinners it is not harmless.
A pronounced deficiency causes muscle and bone pain that is worst in the morning. Measured beforehand — above 100 µg (4000 IU) a day over months without checks counts as too much.
The body’s own cortisol reaches its low point around midnight and only rises again towards morning. In exactly that window inflammatory joint complaints are strongest — morning stiffness has a rhythm, not a coincidence.
An underactive thyroid causes muscle stiffness and muscle pain that are clearest in the morning — without a joint being inflamed. The difference shows in how long the stiffness lasts.
Over 30–60 minutes points to an inflammatory cause, under 15 minutes rather to wear. This timing is more valuable for the work-up than any elimination diet.
In inflammatory rheumatism 2–3 g EPA and DHA a day measurably reduced morning stiffness after 3 months. With wear this effect does not show.
