Chronic fatigue / exhaustion

DE FR EN

Tiredness is a signal, not a character trait — and surprisingly often it has a measurable, fixable cause.

Also known as: fatigue · tiredness · low energy · always tired

Persistent tiredness that sleep doesn’t relieve is a very common symptom with many possible causes — usually none dangerous. Common ones are too little or poor sleep, stress and mental strain (including depression), iron or vitamin B12 deficiency, an underactive thyroid, unrecognised high blood sugar or sleep apnoea. Certain medicines cause tiredness too. A few simple blood tests and an honest look at sleep and daily life often already bring clarity. If the exhaustion persists despite all this and is marked by a clear worsening after exertion, ME/CFS may be behind it (separate dossier).

Levers & pathways

studiedReview sleep & recovery

The key first step: enough and regular sleep, breaks, less stress.

associatedCheck a few basic blood values

Iron/ferritin, B12, TSH and blood sugar clarify many common causes.

associatedTreat the identified cause

Once the cause is found, the tiredness usually improves with its treatment.

associatedConsider ME/CFS on post-exertional crashes

If everything worsens markedly hours after exertion, see the ME/CFS dossier.

traditionalDaylight in the morning

Within the first hour after getting up, 15 to 30 minutes outdoors — even under a cloudy sky.

traditionalAfternoon slump

Lie down for twenty minutes or go outside, no longer sleep after 3 p.m.; keep lunch lighter.

studiedTiredness from a silent blood loss

A worm that entered through the skin while barefoot takes small amounts of blood daily. The iron store empties over months without blood ever being visible.

studiedTiredness after a journey is looked at differently

Leaden tiredness with pallor in the months after a tropical journey can come from the breakdown of red cells. A migrating worm larva also causes tiredness with raised eosinophils — a blood test tells them apart. The migrating larva usually comes from the roundworm of dog and cat, from a sandpit or dog faeces.

infoNumbers and timeframes

About one in five people report lasting tiredness; in roughly two thirds a graspable cause turns up in the end — most often sleep, iron, thyroid, mood or a medicine. It counts as chronic from six months. After an infection, exhaustion lasting four to twelve weeks is the normal course; only beyond that does a wider work-up pay off.

infoWhat the medicine does — and what it does not

There is no drug against tiredness. What is prescribed replaces a documented shortfall: iron works with low ferritin even without anaemia and needs four to twelve weeks; B12 corrects the blood in weeks and the nerves in months; thyroid hormone needs six to eight weeks for full effect. Caffeine moves tiredness, it does not remove it — a half-life of about five hours explains why a coffee at four in the afternoon still disturbs sleep.

studiedAt the wheel: coffee and twenty minutes of sleep

Caffeine needs about twenty minutes. Sleeping briefly beforehand gives both at once. An open window and loud music only fake alertness.

Possible complaints

associatedSleep & stress first

Lack of sleep, chronic stress and mental strain are the most common causes.

associatedIron or B12 deficiency

Detectable via blood tests and usually easy to correct.

associatedUnderactive thyroid

A common, treatable reason for exhaustion.

associatedBlood sugar & sleep apnoea

Unrefreshing sleep (apnoea) and high blood sugar also cause tiredness.

Blood values

associatedFerritin (iron stores)

Iron deficiency is a very common, correctable cause of tiredness.

associatedVitamin B12

A deficiency causes tiredness and can affect the nerves.

associatedTSH (thyroid)

An underactive thyroid slows metabolism and causes tiredness.

associatedHbA1c / blood sugar

Unrecognised high blood sugar can cause ongoing tiredness.

Nutrients

studiedIron

The most common single reason for exhaustion without a finding, above all in women with periods. Ferritin shows the empty store long before the haemoglobin falls — so a normal blood count is no answer here.

studiedVitamin B12

Causes tiredness, problems concentrating and tingling, often already at values in the lower normal range. Under acid blockers, under metformin and with a plant-based diet it belongs checked.

associatedVitamin D

A lack causes muscle weakness, muscle pain and a leaden tiredness. At our latitudes it is the rule from October to April, not the exception.

associatedFolate

Judged together with B12: supplementing it alone can improve the blood count while a B12 deficiency goes on working at the nerves.

studiedFluid

Even a slight fluid deficit lowers concentration and drive measurably. The simplest test takes three days: drink evenly and see whether anything changes.

Supplements

studiedIron

The most commonly findable reason, especially in women before menopause. Ferritin under 30 µg/l counts as the line for many, even without anaemia. 100 mg every second day is absorbed better than daily — otherwise hepcidin blocks it.

mixedVitamin D3 (+K2)

In winter a large part of central Europe sits below the target value. 20–50 µg (800–2000 IU) a day is the usual frame; the studies on tiredness itself are inconsistent, those on bone are not.

studiedVitamin B12

With vegan eating, on metformin and on acid blockers for years it belongs measured. After starting to take it the blood value says nothing — hence beforehand.

mixedCoenzyme Q10 / ubiquinol

Statins measurably lower the body’s own level. Whether that explains the muscle complaints is open; studies at 100–200 mg a day over 12 weeks contradict each other.

infoFolic acid / folate

In pregnancy clearly established against neural-tube defects, otherwise mainly with deficiency. Can mask a B12 deficiency — watch both.

Hormones

studiedThyroid axis (TSH)

The first axis with any lasting tiredness — it is common, simple to check and readily treatable.

studiedCortisol

Too little causes weakness, nausea and low blood pressure; too much causes sleeplessness and exhaustion at the same time. A single value in the afternoon answers neither question.

associatedMelatonin

If the tiredness lies above all in the morning and vanishes in the evening, it is about rhythm rather than a shortage. Light in the morning is then the most effective measure.

associatedInsulin

Tiredness one to two hours after eating, especially after a lot of carbohydrate, belongs to this axis and can be tested by changing what the meal is made of.

Food

studiedFerritin, B12, vitamin D and thyroid

Four values explain a large share of exhaustion and all are treatable. Ferritin below 30 counts as deficiency even when the blood count looks normal — after correction improvement takes 8–12 weeks.

studiedBlood sugar through the day

The afternoon dip after a very carbohydrate-heavy meal is real and measurable. Protein and fibre alongside keep the curve flatter — noticeable within days, not weeks.

studiedCaffeine postpones tiredness, it does not remove it

Caffeine blocks the tiredness signal for some hours; afterwards it returns all at once. In chronic exhaustion this easily becomes a circle that worsens sleep further.

Alternative approaches — what people use

studiedVitamin D

One of the values looked at first with lasting tiredness.

studiedIron

An empty store causes tiredness long before anaemia shows — measure ferritin, do not guess.

traditionalCoenzyme Q10

Much used for exhaustion, especially under statins; the studies are mixed.

traditionalAdaptogens (ashwagandha, rhodiola, ginseng)

Rhodiola and ginseng are traditionally taken against exhaustion.

Wild herbs — what is traditionally used

traditionalStinging nettle

The classic spring course; it replaces no search for the cause.

traditionalRosehip (dog rose)

Rich in vitamin C and keeps well through the winter.

Essential oils — use and limits

mixedPeppermint

The smell makes one more awake, measurably and briefly. Against an iron deficiency, a low B12 or too little sleep it achieves nothing — there the way leads through a value.

infoLemon

Common in the diffuser in the morning. Citrus oils make skin light-sensitive — on the skin only in the evening or in the phototoxin-free quality.

TCM — how it is seen there

traditionalST36 · Zusanli

The main strengthening point in TCM; often warmed with moxa.

traditionalAstragalus (Huang Qi)

Huang qi is the TCM plant for weak qi and frequent infections.
