Oral thrush is an overgrowth of yeasts — usually Candida albicans — that live in almost everyone’s mouth anyway. The best-known picture is a whitish, curd-like coating on reddened lining that wipes off; underneath it is red and raw. There is also a form with no coating at all, where the lining is merely red and burning — under a denture that is in fact the commoner picture. Cracked mouth corners, a furry feeling and an altered taste often come with it. The real question is rarely the yeast but what made room for it: an inhaled steroid spray without rinsing, a course of antibiotics, little saliva, a poorly fitting denture, raised blood sugar or a shifted immune situation. If the burning moves into the throat or swallowing hurts, that belongs with a doctor — the oesophagus can be involved.

Levers & pathways

studiedHave it looked at first, treat second

The appearance is the information: wipeable or not, with coating or without, how far it reaches. Rinsing, scraping or dabbing beforehand takes from the visit exactly what makes it short.

studiedRinse the mouth after the steroid spray

Inhaled steroid is one of the commonest causes of all, and rinsing is almost always forgotten. Rinse and spit after every puff; a spacer further cuts what stays in the mouth.

studiedDenture out at night and cleaned

Under a denture it is warm, damp and undisturbed. Taking it out at night and cleaning it separately is often the real lever in the form without coating — otherwise it returns.

studiedLook for the cause of the dry mouth

Little saliva is rarely “just so”. Often medication is behind it, otherwise mouth breathing, drinking too little, or radiotherapy to the head. While the dryness stays, the ground stays.

studiedHave the blood sugar checked too

Recurring thrush with no obvious trigger is one of the ways a previously unnoticed raised blood sugar comes to light. One value, no effort.

studiedOn a plant-based diet, take the three values along

Iron, B12 and folate belong at the same appointment, not in a second round. They explain the burning independently of the yeast — and where both coincide, otherwise only half gets treated.

studiedIf it returns, ask why

One episode with an obvious trigger is one thing. If it comes back without one, the repetition is itself the finding — then it is not about the next remedy but about the conditions.

mixed“Candida diet” — what it can and cannot do

Widely avoiding sugar and yeast is often advised. For the gut the data are thin. In the mouth what matters is more what sticks to the teeth and for how long — sugary drinks spread across the day are a different thing from the same amount at one meal.

Possible complaints

associatedWhite and wipeable

Curd-like, on tongue, cheek or palate. Underneath the lining is red and raw, sometimes lightly bleeding. That the coating wipes off is the key distinguishing feature.

associatedRed and raw with no coating at all

The second form shows no coating at all, only a reddened, burning area. Under a denture this is the commoner picture — and it is often taken for a pressure sore.

associatedBurning, a furry feeling, altered taste

Many describe it as after a drink that was too hot. Dryness, a furry mouth and sometimes a metallic taste come with it.

associatedCracked mouth corners

Red, sore cracks at one or both corners. They often belong to the picture and rarely settle on their own while the cause remains.

studiedWhen it moves into the throat

Pain on swallowing, a feeling that food sticks, or burning behind the breastbone: the oesophagus may be involved. That is looked at promptly rather than waited out.

infoWhat looks similar and is not

A white patch that does not wipe off is not thrush — lichen planus, leukoplakia and other changes look alike and are assessed differently. A smooth, reddened tongue with no coating points more to a nutrient deficiency than to yeast.

Blood values

studiedHbA1c (long-term sugar)

Raised blood sugar is among the commonest grounds for recurring thrush. With no other explanation to hand, this value is worth measuring.

studiedFerritin (iron store)

Iron deficiency makes the lining more vulnerable and burns in its own right. On a plant-based diet the value is worth having anyway.

studiedVitamin B12

A deficiency gives a smooth, burning tongue — a different picture from the coating, but both can be present at once. On a vegetarian or vegan diet this is the most obvious value.

associatedFolate

To check alongside B12 and iron; the three show up on the lining in similar ways.

associatedZinc

Involved in the lining and in defence. The blood level maps the store only roughly — it is a hint, not proof.

associatedBlood count

Gives the frame: anaemia, inflammation, immune situation. For a one-off thrush with an obvious cause it is not strictly needed.

Nutrients

studiedIron

Deficiency thins the lining and makes it more sensitive. On a meat-free diet the commonest tangible point.

studiedVitamin B12

Needed for renewal of the lining; deficiency shows early on the tongue.

associatedFolate (B9)

Belongs to the same group as B12 and iron and is judged together with them.

associatedZinc

Involved in wound healing and defence. On a plant-based diet absorption is lower because of phytates.

studiedSaliva

The mouth lining’s main protection, and the most overlooked. Little saliva — from medication, mouth breathing or drinking too little — is one of the strongest openers.

Supplements

infoIron

Only with a proven deficiency and specifically. Without one it does nothing, and too much is harmful.

infoVitamin B12

Clearly worthwhile and well supported when deficient. On a vegetarian or vegan diet a topic anyway.

infoFolic acid / folate

When deficient; can mask a B12 deficiency, so look at both together.

infoZinc

With a documented deficiency. Taken high and long term it disturbs copper uptake.

mixedProbiotics

For the mouth there are individual studies with small effects, mainly in older people and denture wearers. As a replacement for treatment it has not been studied.

Alternative approaches — what people use

traditionalChamomile

As a rinse, traditional for irritated mouth lining. Meant to soothe, not aimed at the yeast.

traditionalPropolis

Traditionally used in the mouth; small studies on oral lining exist. Not suitable with an allergy to bee products.

mixedProbiotics

Most studied in older people and denture wearers; the effects are small and the studies few.

traditionalTea tree oil

Used against yeasts and active in the test tube. In the mouth: do not swallow, only well diluted, not in children. Human studies for the mouth are few.
