Recurrent heavy sweating at night that isn’t simply due to a too-warm bedroom. Usually something harmless or treatable is behind it: menopause (very common), an infection, certain medicines, an overactive thyroid, nocturnal low blood sugar, or stress and anxiety. A too-warm bed, alcohol or spicy food in the evening can also trigger it. If the night sweats persist or come with fever, unintended weight loss or swollen lymph nodes, a medical check is warranted — rare then, but important to safely rule out more serious causes.

Levers & pathways

associatedIf it comes by day and in particular places

Sweating that is localised to hands, feet or armpits, equal on both sides and stops during sleep is something other than being soaked at night. It has effective treatments of its own.

associatedFever with no infection in later life

Night sweats, fever and weight loss with no evident reason are, in older people, one of the classic routes to an arteritis — even when head and temple do not hurt at all.

studiedCheck the bedroom & evening

Sleep cooler, less alcohol and spicy food in the evening — often enough.

associatedAssess the common causes

Menopause, thyroid and infections can be checked specifically.

associatedReview medicines

Some medicines promote night sweats — have them reviewed.

associatedWarning signs → doctor

With persistent night sweats plus fever/weight loss, don’t wait.

studiedFever, night sweats and a large spleen

The internal form causes fever, weight loss and a palpably large spleen over weeks. It also comes from the Mediterranean region, not only the tropics, and is treatable.

infoNumbers and timeframes

Night sweats occur at times in about one in three adults. The most common reasons are harmless: an overwarm bedroom, menopause, evening alcohol, medicines. It warrants a work-up when it happens every night for two to four weeks, soaks the bedding and coincides with fever, weight loss or swollen lymph nodes — those three together have a name of their own.

infoWhat the medicine does — and what it does not

There is no drug for night sweats themselves. In menopausal complaints, hormones work most strongly, low-dose SSRIs and SNRIs within one to two weeks, and gabapentin mainly at night. If a medicine is the trigger, changing it helps — SSRIs, opioids, tamoxifen and cortisone are the most common. With night sweats together with fever and weight loss, there is no treating but searching: those three together lead the work-up.

Possible complaints

associatedMenopause

By far the most common cause in women around midlife.

associatedInfection or medicines

Feverish infections and some medicines cause night sweating.

associatedThyroid & low blood sugar

An overactive thyroid or nocturnal lows can be behind it.

associatedWarning combination

With persistent fever, weight loss or swollen lymph nodes: get it assessed.

Blood values

associatedTSH (thyroid)

An overactive thyroid can cause night sweats.

associatedCRP (inflammation/infection)

Helps place an infection or inflammation as the cause.

Nutrients

studiedFluid

Fever, sweating and faster breathing clearly raise the need. Drinking enough keeps the mucus thin and the circulation steady — in children and older people it is the most important point of all.

associatedVitamin D

With low levels airway infections occur somewhat more often; correcting a deficiency makes sense, a high single dose during the infection does not act on it.

associatedZinc

Needed by the defence cells. For lozenges at the start of a cold there are hints of a slightly shortened duration; at a high dose over months zinc lowers copper.

studiedProtein

Antibodies and defence cells are protein. After several days with little appetite it is worth at least keeping the protein up — soup with egg, quark, yoghurt, lentils.

Supplements

traditionalSage

Traditionally used against heavy sweating; the studies are small and open-label, the frame is an extract once daily over 4–8 weeks. Night sweats lasting weeks and coming with weight loss belong investigated rather than treated.

mixedBlack cohosh (Cimicifuga)

Where menopause is the explanation: 40 mg extract a day over 3–6 months is the studied frame, the results are inconsistent.

infoVitamin D3 (+K2)

Clearly worthwhile and well studied when deficient; fewer data for the wider uses. Fat-soluble — check blood levels at high doses.

Hormones

studiedOestradiol

The commonest explanation in women between forty and sixty — the swinging, not the low value. It does not explain fever, weight loss or swollen lymph nodes, though.

studiedThyroid axis (TSH)

An overactive thyroid causes sweating at night with a pounding heart and weight loss — and it is the easiest to find of the more serious causes.

associatedAdrenaline

Sweating in attacks with a racing heart, headache and high blood pressure is rare but an illness in its own right — it is looked for through the breakdown products in the urine.

associatedCortisol

Too much disturbs sleep at night and makes one sweat. It also comes from outside — a cortisone treatment belongs in the same consideration.

Food

studiedAlcohol in the evening

Alcohol widens skin vessels and disturbs temperature regulation in the second half of the night. Two weeks without show whether that is the cause — persistent unexplained night sweats belong investigated.

associatedSpicy food and late eating

Both raise body temperature in the first hours of sleep. The last large meal 3 hours before lying down is the simplest attempt.

Alternative approaches — what people use

traditionalChamomile & sage

Sage is the traditional choice for night sweating.

studiedSleep hygiene

A cool room and breathable bedding often work more than any remedy.

Wild herbs — what is traditionally used

traditionalMeadow sage

The traditional sweating herb — as a tea or extract in the evening. The studies are small and open-label; the experience is old and wide.

traditionalYarrow

Traditionally used alongside. Night sweats that stay for weeks and come with weight loss belong investigated and not treated with tea.

Essential oils — use and limits

infoCommon sage

The traditional sweating oil — here in a wash or diluted on the skin, not internally. Sage essential oil contains thujone and is not meant to be swallowed.

traditionalCypress

Traditionally used alongside, in an evening wash. What stays for weeks and comes with weight loss belongs investigated.
