Peripheral artery disease

DE FR EN

The leg arteries are narrowed — walking hurts, yet it is exactly the most effective treatment.

Also known as: peripheral artery disease · PAD · intermittent claudication

Atherosclerosis narrows the leg arteries, so muscles get too little blood when walking. Typical: cramping calf pain after a set distance that eases on standing. A sign of general atherosclerosis that also raises heart-attack and stroke risk.

Levers & pathways

studiedThe same narrowing, a different area

Anyone who has to stop while walking often has the same deposits on the belly vessels too. Pain after eating with weight loss is the counterpart there to the shop-window disease.

associatedOne vessel stands for all

Narrowed leg arteries almost always mean the coronary arteries are affected too. After this finding it is therefore about the overall risk and not only the leg.

studiedReplace saturated & trans fats

The strongest dietary lever for LDL: replace saturated fats (fatty meat, butter, palm oil) and trans fats with unsaturated ones (olive oil, nuts, fish).

studiedSoluble fibre (oats, legumes, psyllium)

Soluble fibre binds bile acids in the gut; the body pulls LDL from the blood to replace them. Well established and dose-dependent.

studiedPlant sterols/stanols (~2 g/day)

They block cholesterol absorption in the gut and lower LDL by about 10 %. In fortified products or as a supplement.

studiedWhat the medicine does — and what it does not

Statin and antiplatelet protect heart and brain — they change little about walking distance. What actually lengthens it is structured walking training: walk to the pain, stop, walk on, three to five times a week over three months. In many people it matches an angioplasty.

infoNumbers and timeframes

An ankle-brachial index below 0.9 counts as abnormal, below 0.5 as marked; above 1.3 points to stiff vessels and cannot be used. Walking training of thirty to forty-five minutes, three times a week over twelve weeks, doubles to triples the pain-free walking distance in reviews — more than any drug. One walks to moderate pain, pauses, then continues; that stimulus is exactly what builds bypass vessels.

studiedWalking training — the best-evidenced lever

Walk to just before the pain, stop, walk on again — about three times a week over several months. In studies this structured walking training clearly lengthens the pain-free walking distance, and supervised it works better than alone at home. It does not replace the vascular assessment; it stands alongside it.

Possible complaints

studiedCalf pain when walking that eases on standing · cold, pale feet · poorly healing wounds · absent foot pulse · late: rest pain

Rest pain and non-healing wounds are red flags — seek care. Walking training, quitting smoking and risk-factor control are central.

Blood values

associatedApoB / LDL

Drives the arterial narrowing.

associatedHbA1c

Diabetes is a major risk factor.

Nutrients

associatedOmega-3 fatty acids

They lower the blood fats and act on the excitability of the heart muscle. Oily sea fish twice a week is the best-supported route; very high doses from capsules are not a harmless more.

studiedPotassium

Keeps the rhythm steady and lowers blood pressure. Water tablets drive it down, certain blood pressure drugs and a weak kidney drive it up — which is why it is checked under treatment.

associatedMagnesium

A lack favours extra beats and at the same time keeps a potassium deficiency going. Anyone replacing only the potassium sees no success while the magnesium is missing.

studiedSalt

Less salt lowers blood pressure measurably, and most of it is not in the salt cellar but in bread, cheese, cured meat and ready meals. With very low blood pressure the opposite applies.

Supplements

mixedNitrate / beetroot juice

In small studies beetroot juice before exercise lengthened the pain-free walking distance. Walking training remains the measure with the biggest numbers — three times a week to the pain threshold, over at least 12 weeks.

mixedGinkgo biloba

Used for this over decades: 120–240 mg a day over 6 months. The pooled studies show the walking distance lengthening by a few metres — too little to count in everyday life.

infoOmega-3 (EPA/DHA)

Effects on inflammation and triglycerides established; cardiovascular prevention is mixed. Watch quality and freshness.

infoMagnesium (glycinate/citrate)

Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.

Hormones

studiedInsulin

With diabetes the narrowing tends to affect the small lower-leg vessels, and nerve damage takes the warning pain away. This is why exactly the sign otherwise looked for is often missing — the feet then belong looked at regularly, not waited on.

associatedT4 (thyroxine)

An underactive thyroid raises LDL and thereby speeds the deposits. It is one of the few points in this condition that a single value clarifies and a single tablet fixes.

Food

studiedMediterranean diet

Part of vascular prevention.

Alternative approaches — what people use

studiedMovement & exercise

Walking training is the best-evidenced treatment in claudication — walk to the point of pain, pause, walk on. Studied at 30–45 minutes three times a week over 3 months; the pain-free walking distance often doubles.

studiedDietary change

The same vessels as at the heart — the Mediterranean pattern is the best-studied frame here too. Stopping smoking, however, works more strongly than everything else combined.
