Prediabetes means: blood sugar sits above the usual but still below the threshold at which one speaks of diabetes. This is not half an illness but the point with the greatest leverage — the road can often still be left here. Three routes lead to the finding: fasting glucose, HbA1c as a look back over about three months, and the glucose tolerance test, which is the most sensitive and picks up disturbances the other two miss. It matters that a single value decides nothing; confirmation comes on a second day. What happens in the body starts long before: the cells respond less well to insulin, the pancreas makes up for it with more insulin, and only when it can no longer keep up does sugar rise measurably. So the diagnosis is late while the process is old. Not every prediabetes becomes diabetes — some stays put for years, some returns to normal. What makes that more likely is well studied and unspectacular: weight, movement, sleep. Large programmes have shown that guided lifestyle change slows progression more strongly than the usual first medicine — and the effect lasts years beyond the programme.

Levers & pathways

associatedRarely alone

A raised fasting sugar almost never stands alone. Waist, blood pressure, triglycerides and HDL belong measured alongside — three out of five make a picture of its own with levers of its own.

studiedCut sugar & fast carbs

Less sugar, white flour and sugary drinks lowers spikes and, over time, HbA1c most directly.

studiedPost-meal walk

Even 10–15 min of walking after a meal markedly lowers the spike — muscle pulls glucose from the blood.

studiedLose weight

Less (especially visceral) fat strongly improves insulin sensitivity — often the single most effective lever.

studiedConsider a glucose tolerance test

It finds disturbances that fasting glucose and HbA1c miss. More effort, but the clearest answer where there is suspicion.

studiedGuided rather than alone

What worked in the large programmes was not the advice alone but the accompaniment over months. Worth asking for.

studiedMove after eating

Ten to fifteen minutes of walking measurably lowers the peak afterwards — more effective than the same time in the morning.

associatedSleep counts

Short nights worsen insulin action within days. Looking only at food misses this part.

studiedRe-check once a year

Not as a threat but to see whether what was done is working. A value that moves motivates more than any admonition.

studiedNumbers and timeframes

About one adult in three over forty-five sits in the pre-diabetes range. Without change, roughly five to ten in a hundred a year go on to diabetes — with movement and weight loss that halves. It is one of the few figures where one’s own share is this large.

infoWhat the medicine does — and what it does not

Metformin lowers the liver’s sugar output and, over three years, cut the move into diabetes by about a third in trials. A change in food and movement achieved roughly twice that over the same period — one of the few places where the way of living clearly beats the drug. Neither removes insulin resistance; it returns when the trigger returns.

Possible complaints

studiedNothing to feel

Prediabetes causes no symptoms. Waiting for a sign means waiting for diabetes — so it is only found when someone looks.

studiedA waist that grows

Fat around the organs acts most strongly on insulin action. A tape measure says more here than the scales.

associatedDark, velvety skin in the folds

On the neck, in the armpits. It looks like dirt and does not wash off — one of the few visible signs of insulin resistance.

studiedGestational diabetes in the history

It clearly raises the later likelihood. After the birth, sugar belongs checked regularly — which is often forgotten.

studiedFamily with type-2 diabetes

Both predisposition and habits are handed on. That is not a verdict but a reason to measure earlier.

Blood values

studiedHbA1c

Grey zone 5.7–6.4%.

studiedFasting glucose

Mildly raised.

associatedInsulin

Often raised (insulin resistance).

Nutrients

associatedMagnesium

Low levels linked to insulin resistance.

traditionalChromium

Traditional for glucose; there are few studies on it.

Supplements

mixedBerberine

In studies it lowers fasting sugar and HbA1c about as much as metformin — 500 mg two to three times daily. The studies are mostly small and from one region; stomach and bowel complaints are common, and it interferes with the breakdown of many medicines.

mixedChromium (picolinate)

Much promoted for sugar metabolism. Reviews find very small effects on blood sugar at most, and only with a proven deficiency — which is rare in Europe.

infoMagnesium (glycinate/citrate)

Effective when deficient; effects on sleep and stress plausible but moderate. Too much causes loose stool.

Hormones

studiedInsulin

Works less well — the core issue.

Food

studiedLow glycaemic load, high-fibre

The most effective lever.

Alternative approaches — what people use

studiedMovement & exercise

In the large prevention trial lifestyle worked better than the medicine — that is the core finding.

studiedPrebiotics (fibre)

Fibre flattens the sugar curve after the meal.

studiedIntermittent fasting

One possibility among several; the benefit comes mainly through the total amount.
