I am full after a few bites

Being full early is something other than having no appetite. The appetite is there, the meal starts willingly — and after a few bites the room runs out. That turns attention to the stomach and its surroundings. Most often a sluggish stomach lies behind it, emptying more slowly than usual; this occurs with long-standing diabetes, after certain operations, under some medicines and sometimes without a recognisable reason. A functional dyspepsia gives the same picture without a structural finding. Pressure from outside on the stomach — a large liver, fluid in the abdomen, a mass — reduces the room as well. Marked constipation can take room from below. Anxiety and tension act directly on the stomach muscle. In older people emptying slows a little anyway. The combination is decisive: if weight is also being lost, vomiting appears, swallowing is difficult or there is blood in the stool, that should be assessed promptly rather than watched.

Levers & pathways

studiedIf it is not the stomach but the supply that is tight

Early fullness out of experience — because larger portions reliably cause pain — looks like a stomach problem and is not one.

associatedEat smaller and more often

Five to six small meals fill the day without overloading the stomach.

associatedDrink between meals

Drinking with the meal takes the room needed for food.

associatedNote the weight weekly

Same scale, same weekday. The trend matters more than any single number.

associatedSoft, energy-dense food

If little goes in, that little should contain a lot.

infoWhat the medicine does — and what it does not

Prokinetics such as metoclopramide and domperidone speed up stomach emptying — they work within days and are meant for a limited time because of side effects: metoclopramide for at most five days. Low-dose erythromycin also works, but loses effect after weeks. What works independently: small meals, less fat and fibre, fluid between rather than with meals. Early fullness with weight loss is investigated before it is treated.

infoNumbers and timeframes

The stomach holds about fifty millilitres when empty and stretches to one to one and a half litres. A mixed meal is half gone after about ninety to a hundred and twenty minutes; fat slows that considerably. Early fullness together with weight loss over two to four weeks is investigated. In gastroparesis, emptying at four hours is more than ten per cent incomplete — that is the measure used in the scan.

Possible complaints

associatedAppetite yes, room no

This separates early satiety from loss of appetite — two different routes.

associatedWith burping and nausea

The pattern of delayed stomach emptying.

associatedThe belly is tight and large

Pressure from outside on the stomach — that should be examined.

associatedWith weight loss or vomiting

This combination is not watched but looked at promptly.

Blood values

associatedHaemoglobin (blood count)

Anaemia belongs to the basic work-up when intake is low or weight is missing.

associatedFerritin (iron store)

Shows the store before the blood count reacts.

associatedCRP (inflammation)

A simple companion value when weight is being lost as well.

associatedHbA1c

Long-standing high blood sugar slows stomach emptying.

Nutrients

studiedProtein first

If only a little goes in, the order decides: protein first, then the rest. That keeps the muscle mass even when the portion is small.

studiedFluid between the meals

Drinking with the meal fills the already tight space. The drinks belong half an hour before or after — that alone noticeably increases the portion for many.

associatedVitamin B12

With a stomach illness, after a stomach operation and under acid blockers it is taken up less well. That belongs checked if the early fullness lasts.

associatedIron

Taken up in the upper small bowel and needing stomach acid. An empty store despite normal eating points to a problem of uptake — not to too little iron on the plate.

Supplements

mixedGinger

Speeds stomach emptying slightly and is best studied against nausea — 1 g a day, spread out. With sluggish emptying that is the lever with the most data.

infoPsyllium husk

Here as a warning: bulking fibres slow stomach emptying further. Anyone full after a few bites needs denser small meals, not more volume.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

infoIron

Clearly effective with proven deficiency, useless without. Take only with a documented deficiency — excess is harmful.

Hormones

studiedCCK (cholecystokinin)

Released in the small bowel with fat and protein, it reports fullness to the brain while at the same time slowing the stomach. That is the normal road — it only becomes notable when a few bites are enough.

studiedGLP-1

Slows the emptying of the stomach strongly. Drugs from this group cause exactly this feeling — with new, quick fullness the medication list belongs at the start.

associatedGhrelin

Missing hunger before the meal is something other than quick fullness during it. The distinction is worthwhile, because it points to different causes.

Food

studiedSmall meals and drinking between meals

If the stomach empties slowly, fluid at the meal takes up the space. Drinking 30 minutes before or after rather than with the meal makes room for food.

studiedFat and fibre delay further

Both slow stomach emptying. With early fullness eating is therefore temporarily lower in fat and fibre — the exception to the usual rule.

Alternative approaches — what people use

mixedGinger

Ginger slightly speeds stomach emptying — the fitting approach with early fullness from a sluggish stomach. If the feeling lasts or weight falls, the stomach belongs looked at.
