Dysphonia
Hoarseness lasting more than three weeks belongs in a laryngoscopy — and whispering does not rest the voice.

The voice is a working instrument — and it can be overloaded like any other. Specialists distinguish two large groups. Functional voice disorders arise from speaking too much, too loudly or with unhelpful technique: teachers, childcare staff, singers, call-centre workers, sales staff and anyone who has to talk over noise are especially affected. The larynx looks unremarkable at first, yet the voice sounds rough, pressed, breathy and tires over the day. Organic voice disorders have a visible cause: vocal fold nodules from sustained overload, polyps and cysts, vocal cord paralysis after thyroid or chest surgery, inflammation, reflux from the stomach or the consequences of smoking. The two groups shade into each other — poor technique eventually changes tissue. One rule holds without exception: hoarseness lasting longer than three weeks belongs in a laryngoscopy with an ENT doctor — especially for people who smoke and drink regularly, because laryngeal cancer can hide behind it and is very treatable when found early. Two common habits make things worse: whispering does not spare the voice, it strains it; and constant throat clearing is a vicious circle that irritates the lining further. Treatment is voice therapy with a speech therapist, prescribed by a doctor and covered by basic insurance. How this is worked on in speech therapy — amount, duration, limits and the signs that are urgent — is ordered by age at logopaedie.borters.ch.
The ENT doctor looks at the vocal folds. Especially with smoking and regular alcohol — early cancer is very curable.
Breath, posture, resonance and technique can be trained — nodules often recede with it.
Whispering does not spare the voice, it squeezes it. Better to speak softly but with normal voicing.
Instead of clearing: swallow, sip water or cough lightly — clearing irritates and produces more mucus.
Dry heated air and smoke are the mucosa’s toughest enemies. Stopping smoking has the biggest effect.
Avoid late heavy meals, raise the head of the bed and seek medical advice if the morning voice stays rough.
Do not shout over noise, use a microphone, plan vocal breaks, get attention by means other than volume.
Speaking less for a few days helps; total silence for weeks actually weakens the voice further.
Cortisone makes the voice more resilient for hours to days and is used before performances — it takes the warning away while the strain continues and can thereby increase the damage. For functional voice disorder there is no drug; voice therapy over eight to twelve sessions is the route with the most data. With nodules the tissue recedes under therapy over weeks to months, without surgery.
The vocal folds vibrate in speech about a hundred to a hundred and twenty times a second in men and two hundred to two hundred and fifty in women — a female singer exceeds a thousand. Hoarseness lasting more than three weeks calls for a look at the larynx. After acute overuse the voice needs three to seven days. Voice therapy runs to eight to twelve sessions. Whispering strains the folds more than quiet speech — the most common well-meant mistake.
All the voice signs in one place: quiet and monotone, deep and rough, breathy after neck surgery, pressed in speaking professions — with what tells them apart.
The sound turns scratchy or breathy, volume cannot be held, high notes go first.
Fine in the morning, gone by afternoon. Typical of voice professions — it recovers over the weekend.
A lump or foreign-body feeling plus constant throat clearing — which irritates the lining and worsens it.
After thyroid or chest surgery a vocal nerve may be injured — always report it.
The firm line: hoarseness lasting over three weeks needs an ENT laryngoscopy — all the more so if you smoke.
Night-time reflux irritates the larynx — often without classic heartburn, but with a rough morning voice.
The thyroid changes the sound and the stamina of the voice. The value belongs at the start, before long practice.
An iron shortage lowers overall stamina and with it that of the voice. That is no proof, but a worthwhile look in speaking professions.
Gums that bleed at the lightest touch are among the first signs of a lack. It is rare today but occurs with a very one-sided diet and with alcohol.
Recurring mouth ulcers, a smooth burning tongue and cracked corners of the mouth point to a lack — the mouth often shows this before the blood count does.
The jawbone that holds the teeth undergoes the same building up and breaking down as any other bone. What counts for the hip counts for it.
Saliva is the natural protection of teeth and lining. A dry mouth — from medicines, mouth breathing or drinking too little — raises caries, bad breath and fungal growth all at once.
The vocal folds need a moist surface in order to vibrate. Dry heated air, little drinking and much throat-clearing dry them out — and throat-clearing is the most harmful of the three, because it slaps the folds together. A sip of water instead of a throat-clear is the single most effective measure.
Sage lozenges are popular and subjectively feel good. On the vocal folds themselves that does not act — what helps is the flow of saliva from sucking, and any sugar-free lozenge brings that as well. Menthol dries rather than moistens.
Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.
Around the period and in menopause the voice changes measurably — the vocal cords take up fluid and vibrate differently. Singers know this as the “premenstrual voice syndrome”.
Inhaled cortisone for asthma makes the voice hoarse in some — through a fungal colonisation or directly at the muscle. Rinsing the mouth after inhaling and using a spacer solves most of these cases.
Silent reflux reaches the larynx without causing heartburn — morning hoarseness and throat clearing are typical. Eating late and large evening meals are the most common amplifier.
The voice rests on the breath. Speech therapy breathing and voice exercises are the best-evidenced treatment; whispering, incidentally, does not spare the voice, it strains it further.
