Toothache is a clear signal not to ignore — usually a well-treatable cause is behind it. The most common are cavities or exposed, sensitive tooth necks; if the pain grows or the tooth throbs, the nerve may be inflamed. The gum, a filling or a wisdom-tooth situation can also hurt. Sometimes the pain radiates from the sinus or the jaw joint into the teeth, though the teeth themselves are healthy. Short-term relief is possible, but the real solution lies in a dental check. A warning sign is swelling of the face or jaw with fever — that points to an abscess and needs prompt care.

Levers & pathways

associatedWhen no tooth is the reason

Lightning pains lasting seconds, set off by touch, chewing or a draught, with complete quiet in between: that is not toothache. This distinction saves root treatments and extractions with no effect whatever.

associatedShort-term relief

Proven pain relievers and cooling from outside can bridge to the appointment.

associatedDon’t delay the dentist

Only the dentist clarifies and treats the cause.

associatedAvoid triggers

Until the visit, avoid very hot, cold and sweet on the affected side.

associated⚠️ Swelling/fever quickly

With swelling, fever or feeling very unwell, don’t wait.

traditionalCooling from outside for toothache

Hold a cooled cloth against the cheek from outside for 15 minutes, with breaks; sleep with the head raised.

infoNumbers and timeframes

A pain that shoots in briefly with cold and stops after seconds points to a sensitive tooth; a pain that stays a minute or more after the stimulus, or wakes you at night, points to an inflamed nerve. That difference in timing decides whether a filling is enough or a root treatment is due. An abscess pressing outward needs treatment within hours; painkillers bridge the gap, they end nothing.

infoWhat the medicine does — and what it does not

Painkillers bridge the time until treatment — they do not reach the inflamed nerve inside a closed tooth, because the pressure stays within. Anti-inflammatories usually work better here than paracetamol, because prostaglandins are involved; the two together work more strongly than either alone. An antibiotic does not replace treatment: it holds back a spread but opens no drainage. A tablet laid on the gum burns the lining instead of helping.

studiedUntil the appointment: cool and keep the head up

Cool from the outside, keep the head raised at night too — lying down raises the pressure, which is why it often hurts more at night. Never place aspirin on the gum.

Possible complaints

associatedCavities & sensitive teeth

Most common cause; sensitive to sweet, hot or cold.

associatedInflamed tooth nerve

Throbbing, worsening pain can point to nerve inflammation.

associatedGum & referred pain

Gum, sinus or jaw joint can radiate into the teeth.

associated⚠️ Swelling with fever

Face/jaw swelling with fever suggests an abscess → get treated quickly.

Blood values

studiedCRP

Toothache alone needs no blood values. If swelling of the face, fever or difficulty swallowing come with it, a spread is at issue — and then every hour counts.

associatedBlood count

Belongs to the same question. An abscess at the jaw can spread quickly into the soft tissue of the neck.

associatedHbA1c (long-term sugar)

A raised sugar encourages inflammation of the gums and abscesses and delays healing after a procedure.

Nutrients

studiedVitamin C

Gums that bleed at the lightest touch are among the first signs of a lack. It is rare today but occurs with a very one-sided diet and with alcohol.

studiedVitamin B12 and iron

Recurring mouth ulcers, a smooth burning tongue and cracked corners of the mouth point to a lack — the mouth often shows this before the blood count does.

associatedCalcium and vitamin D

The jawbone that holds the teeth undergoes the same building up and breaking down as any other bone. What counts for the hip counts for it.

studiedFluid

Saliva is the natural protection of teeth and lining. A dry mouth — from medicines, mouth breathing or drinking too little — raises caries, bad breath and fungal growth all at once.

Supplements

mixedVitamin C

Gums that bleed with every brushing are one of the first signs of vitamin C deficiency — with scurvy the teeth loosen in the end. It does not act on an acute toothache.

mixedVitamin D3 (+K2)

A low level is linked with more decay and more gum inflammation. That is prevention over months, not a remedy for tonight’s pain.

mixedStreptococcus salivarius (K12/M18)

M18 has been studied for plaque and gum inflammation, K12 for bad breath — lozenges over 4–12 weeks. The effects are small and last only while taken.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

Hormones

associatedParathyroid hormone (PTH)

An overactive parathyroid breaks down the jaw bone too — loose teeth without gum disease are a rare but clear sign. A calcium value settles it.

infoAdrenaline

A heart attack can show as tooth or jaw pain, especially in the lower jaw and especially in women and with diabetes. Toothache that comes with exertion and goes at rest does not belong at the dentist.

Food

studiedHow often, not how much

After every sugary bite the pH in the mouth falls for about 30 minutes. Five small snacks across the day harm teeth more than the same amount at once.

studiedDo not brush right after something acidic

Citrus, soft drinks and wine briefly soften the enamel. Wait 30 minutes, rinse with water first — otherwise the softened enamel is brushed away.

Alternative approaches — what people use

traditionalChamomile & sage

Rinses ease irritation of the gums. Clove oil numbs briefly — both bridge the time until the appointment, neither treats the cause. Warm compresses on a swollen cheek can spread the infection.

Essential oils — use and limits

traditionalClove

The traditional remedy for toothache — a drop on cotton wool, not on the gum.

TCM — how it is seen there

traditionalLI4 · Hegu

The best-known point for toothache — pressed on the opposite side.

traditionalST44 · Neiting

On the foot; traditionally for toothache with a hot feeling and swollen gums.
