What matters is when it shakes: while holding, at rest, or only with coffee, excitement and lack of sleep.

Almost everyone has a fine tremor of the hands — you see it when you stretch out your arms, and it grows after coffee, little sleep, excitement or cold. That is the normal physiological tremor. The useful question is when it shakes. Shaking while holding a cup or writing, slowly increasing over years and often running in the family, points to essential tremor — the most common condition in its own right. Shaking at rest, with the hand lying relaxed in the lap, together with smaller handwriting or stiffer movements, is a different pattern and should be assessed. A tremor that is new and unexplained is worth a look at thyroid, blood sugar and medication: an overactive thyroid, a low sugar, asthma inhalers, some antidepressants, cortisone or lithium can all cause shaking. Alcohol has a double role — it damps in the moment and worsens in withdrawal. Strong shaking is not “just nerves”: it can be placed.
If the tremor governs everyday life and medication no longer suffices, deep brain stimulation is among the established possibilities. Here it targets a different point in the brain than in Parkinson’s.
The simplest test: if the shaking clearly drops, much of it was caffeine.
Lack of sleep and long gaps between meals amplify any tremor, whatever the cause.
Arm outstretched, then relaxed in the lap, then writing — this helps more than any description.
With start dates. Many cases resolve through timing alone.
If the tremor appears at rest, starts on one side and the movements slow down with it — unlike the tremor that only comes on holding or reaching.
Essential tremor shows on holding and acting, at four to twelve oscillations a second, and improves temporarily with alcohol in some people — that is a distinguishing feature, not a treatment suggestion. Parkinson’s tremor, by contrast, shows at rest, at four to six oscillations, and disappears with movement. An exaggerated physiological tremor arises from caffeine, thyroid, low blood sugar, alcohol withdrawal and medicines — it is the most common of all.
Beta blockers such as propranolol and primidone have been studied in essential tremor; they halve the amplitude in some people and work within days to weeks. They do not remove the tremor. In Parkinson’s tremor the usual Parkinson drugs work, but on the tremor least of all the signs. Exaggerated physiological tremor needs no drug but the removal of the cause — caffeine, thyroid, alcohol withdrawal, medicines.
Cup, spoon, signature — increasing over years, often familial: the essential tremor pattern.
With smaller handwriting, stiffer movement or reduced facial expression: get it assessed.
Coffee, nicotine, little sleep, excitement, cold, hunger — the normal tremor, amplified.
Asthma inhaler, some antidepressants, cortisone, lithium, thyroid hormone — the timing is the clue.
An overactive thyroid typically causes a fine tremor, with restlessness and feeling warm.
A low blood sugar causes shaking, sweating and hunger — usually in episodes.
Makes sure the muscle lets go again after tensing. A lack favours cramps, twitching and inner restlessness — and the blood value reflects the store poorly.
Sets the excitability of muscle and nerve. A low value causes weakness and cramps, a high one is dangerous for the heart rhythm — here one measures rather than guesses.
Sets off the tensing in the muscle in the first place. A low value causes tingling around the mouth and in the fingers and cramping of the hands.
With heavy sweating both are lost together. Drinking only water dilutes the sodium further — with long exertion salt belongs with it.
With a proven deficiency tremor and muscle twitching improve. Without one nothing moves — 200–400 mg a day over 4 weeks shows whether that was the reason.
Amplifies the fine natural tremor dose-dependently. Four cups a day is one of the most common explanations for shaking hands — and the test is simple: two weeks without.
A deficiency causes tremor and unsteadiness in walking, often together with tingling. Measured before replacing.
A fine, fast tremor of the outstretched hands together with warmth, a racing heart and weight loss is the picture of an overactive thyroid — and treatable.
Explains the trembling with excitement, caffeine and low blood sugar. It is brief, vanishes at rest and has no disease meaning.
Trembling with sweating and ravenous hunger that stops at once after eating belongs to low blood sugar — especially with treated diabetes.
Caffeine clearly amplifies the normal fine tremor. In essential tremor it makes the difference between unnoticeable and disturbing — two weeks without is worth trying.
Trembling with sweating and palpitations during long gaps between meals is low blood sugar. Trembling in the morning that vanishes after the first drink is withdrawal — the two belong told apart.
Often recommended for tremor. A benefit is shown only with a proven deficiency — in essential tremor it does not work. Leaving out caffeine does more there.
