A cancer of the blood-forming system: in the bone marrow, immature white blood cells multiply uncontrollably and crowd out healthy blood production. This causes the typical complaints — pallor and fatigue (too few red cells), frequent or stubborn infections (non-functional white cells), and bruising, nosebleeds or bleeding gums (too few platelets). Night sweats, fever without infection, unintended weight loss, bone pain and swollen lymph nodes can follow.

Four main forms with very different courses: acute lymphoblastic leukaemia (ALL) mainly affects children and is curable there in most cases. Acute myeloid leukaemia (AML) occurs more often at older ages and needs prompt treatment. Chronic myeloid leukaemia (CML) has been transformed by targeted tablets from a life-threatening into a usually well-controlled condition — many now have a near-normal life expectancy. Chronic lymphocytic leukaemia (CLL) often runs silently for years; watchful waiting without treatment is not unusual at first, which sounds strange to relatives but is medically justified.

Levers & pathways

studiedAsk about the tumour board discussion

The treatment plan is not made by one person alone but in a round of surgery, oncology, radiology, pathology and radiotherapy. You may ask what was discussed there and which alternatives came up.

studiedA second opinion is usual and no sign of mistrust

Before a big decision it is planned for in many clinics, and the records belong to you. It costs one to two weeks — acceptable in almost every course, and one asks about that if it is urgent.

studiedMovement during the treatment

It is today part of the treatment and not its opposite: it lessens the exhaustion, keeps muscle and bone and improves how the treatment is tolerated. Begin with guidance, less on bad days, but not nothing at all.

studiedHolding weight matters more than any diet

Unintended weight loss worsens how any treatment is tolerated. Restrictive diets do more harm than good here — with loss of appetite, dietary advice helps early, not only once kilos are already gone.

studiedTreat pain early and regularly

By the clock rather than as needed works better and needs less overall. Fear of dependence is the wrong adviser in this situation — untreated pain costs sleep, appetite and strength.

studiedTake support along from the start

Psycho-oncological support, social advice for insurance and work, and the question of who takes on what at home. This is no emergency measure but belongs at the start — and the relatives belong in it.

studiedFever during treatment is an emergency

If the white cells are low, a fever above thirty-eight degrees is a reason to go to the clinic at once — not the next day. The department’s emergency number belongs in the phone and on the fridge.

studiedRule out the threadworm before immune therapy

A threadworm can stay silent in the gut for decades and, under cortisone, biologicals or other immune suppression, flood lung and bloodstream. Anyone who ever went barefoot in the tropics or subtropics is looked for beforehand with a blood test — one effort, and it prevents the worst course this worm has.

studiedTreatment: what to expect

For acute forms, chemotherapy in several phases, often over months and with longer hospital stays. For CML, today usually tablets taken at home. A stem cell transplant is an option for some. Between diagnosis and starting treatment there are sometimes days of tests — it feels endless, but it serves to pin down the exact subtype, and the whole treatment depends on that.

studiedWhy infection protection becomes so important

During treatment, immune cells drop sharply at times. A trivial infection can then become dangerous, and fever is an emergency to report at once — do not wait until morning. For relatives, practically: keep your distance if you have a cold, take hand hygiene seriously, and when in doubt call too early rather than too late.

studiedThe exhaustion is part of the illness

The tiredness in leukaemia and under treatment is different from ordinary exhaustion — sleep does not clear it and willpower cannot shift it. It comes from the anaemia, the treatment and the illness itself. Knowing this lifts a burden: it is not weakness and not a failing will to fight. Gentle activity, within what is possible, demonstrably helps more than complete rest.

traditionalFor relatives

The most helpful is often the unspectacular: being there, without encouraging phrases; taking on concrete tasks rather than offering help in general; listening, including about fear. Patients often say they are exhausted from having to comfort others. And: relatives reach their own limits — counselling services and support groups are expressly there for them too. Seeking support is not a betrayal of the person who is ill.

infoWhat the medicine does — and what it does not

Chronic myeloid leukaemia is the example where everything changed: a tablet that blocks exactly the altered protein turns a fatal illness into one you live with — it is taken permanently, and after years of deep remission a trial of stopping can be discussed. In the acute forms treatment runs in blocks over months with cure as the aim. In the chronic lymphocytic form, watching is often the first step, because treating early does not lengthen time.

infoNumbers and timeframes

Acute lymphoblastic leukaemia is the most common cancer in children; more than ninety in a hundred are cured. In acute myeloid leukaemia in adults the figures are considerably lower depending on the subgroup. Chronic myeloid leukaemia today has a nearly normal life expectancy on targeted treatment — before the year 2000 it was a few years. The chronic lymphocytic form is often only watched for years, because treating early does not lengthen time.

Possible complaints

studiedTiredness and pallor that do not go

An exhaustion that builds over weeks and does not improve with sleep, together with pale skin and breathlessness on stairs. Far more often iron deficiency is behind it — the same blood count answers both.

studiedBruises without a cause

Bruises in places where nothing was bumped, small red dots on the lower legs, nose or gum bleeds that will not stop.

studiedInfections that pile up

Repeated fever, sore throats or skin infections at short intervals, often without anyone around being ill.

studiedNight sweats and weight loss

Night clothes soaked without a warm room, kilos lost unintentionally and sometimes bone or joint pain, in children mainly in the legs.

associatedSwollen lymph nodes, spleen or gums

Painless lumps on the neck, in armpits or groin, a feeling of pressure under the left ribs, or noticeably overgrown gums.

Blood values

studiedWhite blood cells

Often markedly raised, in some forms lowered — the differential count with immature precursors is what matters.

studiedHaemoglobin

Falls because healthy blood formation is displaced — hence pallor and fatigue.

studiedPlatelets

Too few platelets explain bruising and bleeding.

studiedNeutrophils

Functional defence cells are lacking — hence frequent infections.

associatedLDH

Rises with high cell turnover; non-specific but often abnormal.

associatedUric acid

Raised from the breakdown of many cells.

Nutrients

studiedProtein

The loss of muscle mass helps decide how well a treatment is tolerated. Enough protein spread across the day, together with movement, is the part one holds oneself.

studiedIron

Where blood is lost regularly the store empties long before the haemoglobin falls. Ferritin shows this earlier than the blood count — and refilling takes months, not weeks.

associatedVitamin D

Low values are common with a longer illness, and correcting them counts for bone and muscle. For high-dose courses against the illness itself there are no studies; they can interfere with treatment.

studiedVitamin C

Clearly improves the uptake of plant iron. A glass of orange juice or some pepper with the iron-rich meal does more than the choice of preparation.

associatedVitamin B12 and folate

Belong in the workup of an anaemia that is otherwise too quickly put down to the illness or the treatment. Both are easy to measure and easy to supplement.

Supplements

infoSt. John’s wort

The most important pointer: in the liver it lowers the level of several cancer medicines, among them imatinib and irinotecan. A plant remedy for mood can weaken the treatment without anyone noticing.

infoProbiotics

With strongly weakened defences and low white cells, live organisms from a capsule are not harmless — bloodstream infections from probiotics are described. That is the one situation where they do not belong.

infoIron

Clearly effective with proven deficiency, useless without. Take only with a documented deficiency — excess is harmful.

infoVitamin D3 (+K2)

Clearly worthwhile and well studied when deficient; fewer data for the wider uses. Fat-soluble — check blood levels at high doses.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

Hormones

studiedCortisol

Cortisone is itself part of the treatment in some forms — in doses far above anything otherwise given. This is why sugar, bone and mood belong cared for from the start.

associatedInsulin

Under high-dose cortisone a temporary diabetes arises in many. It mostly goes again after tapering — in the meantime it belongs treated like any other.

Food

studiedLow-germ food during neutropenia

When white cells are low, raw meat, raw milk cheese, raw eggs and unwashed fruit are left out. Strict germ avoidance is handled more sparingly today than before — kitchen hygiene matters more than the list.

studiedKeeping weight and protein

A sore mouth lining and nausea make eating hard. Cool, soft food, nutritional drinks and several small meals are the usual approach — weight loss worsens tolerance of therapy.

mixedSugar feeds the tumour — what is true

The claim usually rests on the PET scan: weakly radioactive glucose is injected, and the tumour lights up. That shows tumour cells take up sugar faster than the tissue next to them — not that eaten sugar feeds them. You come to a PET scan fasting, and the tumour lights up anyway; the body keeps blood sugar within narrow limits by itself. It cannot be starved out this way. There is an established link nonetheless, but it runs through body fat: in 2016 the International Agency for Research on Cancer (IARC) reviewed the available studies and found sufficient evidence, for thirteen cancers, that absence of excess weight lowers the risk. That concerns prevention — it is not an answer to where an existing illness came from.

Alternative approaches — what people use

studiedMovement & exercise

Even during treatment moderate movement preserves muscle mass and improves fatigue. With very low platelets the intensity is adjusted — this belongs discussed with the treating team.

infoProbiotics

Here as a warning: with severely weakened immunity live bacterial preparations are advised against — cases of blood poisoning are described. One of the few clear contraindications.
