Lymphoma sounds threatening but is often very treatable — some forms are even curable.
Also known as: lymphoma · Hodgkin lymphoma · non-Hodgkin lymphoma · lymph node cancer

Cancer of the lymphatic system — lymph nodes, spleen and immune cells. Typical: painlessly swollen nodes in the neck, armpits or groin that persist or grow over weeks. Plus so-called B symptoms: night sweats that soak the bedding, fever without infection and unintended weight loss; sometimes stubborn itching. Hodgkin lymphomas, often affecting young people and well treatable today, are distinguished from the diverse non-Hodgkin lymphomas. Diagnosis requires removing a lymph node.
The treatment plan is not made by one person alone but in a round of surgery, oncology, radiology, pathology and radiotherapy. You may ask what was discussed there and which alternatives came up.
Before a big decision it is planned for in many clinics, and the records belong to you. It costs one to two weeks — acceptable in almost every course, and one asks about that if it is urgent.
It is today part of the treatment and not its opposite: it lessens the exhaustion, keeps muscle and bone and improves how the treatment is tolerated. Begin with guidance, less on bad days, but not nothing at all.
Unintended weight loss worsens how any treatment is tolerated. Restrictive diets do more harm than good here — with loss of appetite, dietary advice helps early, not only once kilos are already gone.
By the clock rather than as needed works better and needs less overall. Fear of dependence is the wrong adviser in this situation — untreated pain costs sleep, appetite and strength.
Psycho-oncological support, social advice for insurance and work, and the question of who takes on what at home. This is no emergency measure but belongs at the start — and the relatives belong in it.
A threadworm can stay silent in the gut for decades and, under cortisone, biologicals or other immune suppression, flood lung and bloodstream. Anyone who ever went barefoot in the tropics or subtropics is looked for beforehand with a blood test — one effort, and it prevents the worst course this worm has.
In Hodgkin lymphoma and many aggressive lymphomas the aim is cure, not control — that sets them apart from most other cancers. Chemotherapy runs in cycles over four to six months; the antibody rituximab marks the B cells for the immune system. Indolent lymphomas, by contrast, are often only watched at first, because early treatment does not lengthen time — that waiting is a decision, not a delay.
Hodgkin lymphoma affects mainly two age groups: around twenty and over sixty; more than eighty in a hundred are cured. In the aggressive non-Hodgkin forms, treatment runs in six cycles over about four to five months, with cure as the aim. Indolent forms often need no treatment for years. A swollen lymph node lasting more than four to six weeks, hard and painless, is the sign that is followed up.
On the neck, above the collarbone, in armpit or groin, rubbery firm, movable, present for more than four weeks and rather growing. Painful nodes after an infection are the opposite — they belong to the defence.
Fever without an infection, heavy night sweats and unintended loss of more than a tenth of body weight in half a year. Together with a lump, one does not wait.
An itch without a visible rash, often worst at night. It has many harmless reasons — but together with lumps and night sweats it is a reason for an examination.
A dry cough, breathlessness, a tight feeling behind the breastbone or bulging neck veins can come from nodes in the chest. Belly pain and early fullness from nodes or an enlarged spleen.
The loss of muscle mass helps decide how well a treatment is tolerated. Enough protein spread across the day, together with movement, is the part one holds oneself.
Low values are common with a longer illness, and correcting them counts for bone and muscle. For high-dose courses against the illness itself there are no studies; they can interfere with treatment.
Belong in the workup of an anaemia that is otherwise too quickly put down to the illness or the treatment. Both are easy to measure and easy to supplement.
Nausea, diarrhoea and fever dry one out quickly, and several treatments strain the kidney. Drinking enough is no side issue here.
The most important pointer on this page. It speeds the liver breakdown of numerous medicines, among them several cytostatics and immunosuppressants — the drug level falls measurably, in part by half. Anyone receiving treatment discusses it beforehand, not afterwards.
Under cortisone, which is part of many lymphoma regimens, bone density is lost quickly. Vitamin D and calcium accompany here from the start; the amount follows the measured level.
Two sides: with strongly weakened defences — for instance in the phase with very low white blood cells — live organisms in capsule form call for caution; isolated bloodstream infections are described. Outside that phase the reservation does not apply.
Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.
The most important point: cortisone before the biopsy can change the cells so much that no diagnosis is possible — and thereby delay the right treatment. With an unclear lump one looks first and treats afterwards.
Radiation to the neck damages the thyroid — in a large share of those treated an underactive thyroid arises years later. The value therefore belongs in lifelong follow-up.
In children and young adults the treatment also affects growth and fertility. Both belong discussed before it begins — eggs and sperm can be stored beforehand, not afterwards.
Taste changes and nausea are common. Cold food smells less and is often tolerated better; a metallic taste can be eased with plastic cutlery.
The claim usually rests on the PET scan: weakly radioactive glucose is injected, and the tumour lights up. That shows tumour cells take up sugar faster than the tissue next to them — not that eaten sugar feeds them. You come to a PET scan fasting, and the tumour lights up anyway; the body keeps blood sugar within narrow limits by itself. It cannot be starved out this way. There is an established link nonetheless, but it runs through body fat: in 2016 the International Agency for Research on Cancer (IARC) reviewed the available studies and found sufficient evidence, for thirteen cancers, that absence of excess weight lowers the risk. That concerns prevention — it is not an answer to where an existing illness came from.
Supervised training during chemotherapy improves fatigue and stamina. The intensity follows the day and the blood values.
