The immune system attacks the nerve sheaths — today there are many medicines that slow the course.

This compilation is for information and replaces neither medical diagnosis nor treatment. If you have symptoms, see a doctor.

A chronic inflammatory autoimmune disease of the central nervous system: the immune system attacks the myelin sheaths (and axons) in brain, spinal cord and optic nerves. Symptoms depend on lesion location — visual disturbance, numbness/tingling, weakness, gait and balance problems, marked fatigue. Usually starts in relapses, later often progressive. Diagnosis rests on MRI (lesions spread in space and time) plus clinical findings, supported by spinal fluid (oligoclonal bands) — there is no single blood test.

Levers & pathways

associatedOften the first sign

A painful visual disturbance in one eye in younger adults is often the first attack. Knowing that leads earlier to imaging — and so earlier to a preventive treatment.

studiedHeat makes it worse for a while

A hot bath, sauna, fever or a warm summer day bring back complaints that had already gone. That is not a new relapse but a conduction problem that goes again with cooling.

studiedMovement is treatment here

Endurance and strength at a tolerable dose improve walking distance, balance and exhaustion. The old advice to take it easy has long since been dropped.

studiedPlan the exhaustion instead of fighting it

Put the hardest tasks in the best time of day, build in short breaks before the exhaustion comes. Pausing only when nothing works any more costs the whole afternoon.

studiedBladder and bowel belong in the conversation

Frequent urgency, residual urine and constipation are common and are rarely raised spontaneously. They can be treated well — and untreated residual urine leads to repeated infections.

associatedMeasure and set vitamin D

Low values are common in this illness, and correcting them is simple and cheap. How strongly that shapes the course is being studied; the correction itself makes sense regardless.

studiedTreat fever and infections early

A urinary infection or a feverish cold makes existing deficits stand out much more. Bringing the fever down helps more here than in others.

studiedRule out the threadworm before immune therapy

A threadworm can stay silent in the gut for decades and, under cortisone, biologicals or other immune suppression, flood lung and bloodstream. Anyone who ever went barefoot in the tropics or subtropics is looked for beforehand with a blood test — one effort, and it prevents the worst course this worm has.

studiedEpstein–Barr virus (EBV) as a necessary factor

Almost all patients had prior EBV; a large cohort study (2022) points to EBV as a near-necessary but not sufficient factor.

associatedVitamin-D deficiency / low sun / high latitude

Consistent epidemiological association (latitude gradient); a causal contribution is likely but not proven.

studiedSmoking

Established risk and progression factor.

studiedHeat/fever transiently worsens symptoms (Uhthoff phenomenon)

A real but reversible worsening from heat — not a new relapse or new damage. See “Heat intolerance”.

studiedWhat the medicine does — and what it does not

Disease-modifying therapies lower the number of relapses and new lesions. They do not repair existing damage. The earlier they start, the more can be prevented — that is the reason for the hurry at the beginning.

studiedNumbers and timeframes

The disease usually starts between twenty and forty, and women are affected two to three times more often. It becomes more common with distance from the equator. In most people the course is relapsing, and the first years decide most about what remains.

Possible complaints

studiedVisual disturbance/optic neuritis · numbness/tingling · weakness · gait/balance problems · marked fatigue · bladder issues · heat sensitivity

Which symptoms appear depends on lesion location — highly individual.

Blood values

associatedVitamin D (25-OH)

Low levels are associated with higher risk and more disease activity.

associatedVitamin B12

Important for the differential — B12 deficiency can mimic MS-like neurological symptoms.

Nutrients

associatedVitamin D

The strongest nutrition-related link — sun and intake shape the level.

associatedVitamin B12

Central for myelin and nerves — deficiency should be ruled out.

Supplements

associatedVitamin D

Clearly sensible when deficient; as a disease brake the evidence is mixed.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

Hormones

associatedOestradiol

Women are affected 2–3× more often; relapse activity often drops in late pregnancy.

Messengers

studiedMyelin & nerve conduction

MS destroys myelin — the fast, saltatory nerve conduction breaks down.

Food

mixedVitamin D — link established, benefit open

The further a country lies from the equator the more common MS is, and low levels are linked with more relapses. Whether supplementing changes the course has not been clearly shown. Both belong side by side.

mixedSalt, weight and gut flora

Animal experiments showed more inflammation on very salty diets; in humans this is not confirmed. Excess weight in adolescence, however, is linked with higher risk in several cohorts.

no studiesNo proof for the many MS diets

Swank, Wahls, paleo and others are heavily promoted. None has so far shown an effect on relapses or disability in controlled trials. Anyone trying them should watch for nutrient gaps — that belongs to it.

Alternative approaches — what people use

studiedVitamin D

Low levels go together with more frequent relapses; supplement after measuring.

studiedMovement & exercise

Improves strength, walking distance and fatigue — resting worsens all three.

studiedHeat & cold

Heat temporarily worsens symptoms; cooling before movement makes it possible.
