Fat in the liver is a silent but well reversible early sign of metabolism — the liver forgives a lot.

The liver normally stores almost no fat. When it receives more energy than it can process — above all from sugar and fructose — it lays the surplus down inside its own cells. From about five per cent fat content one speaks of a fatty liver. It is today the commonest liver disease of all, and it by no means only affects people who are overweight; slim people can have it too, especially with insulin resistance. What matters is telling apart two states that share a name and behave very differently: plain fat storage, which is largely harmless and well reversible, and fatty liver inflammation, in which cells are damaged and scarring can build over years. Which of the two is present, the liver enzymes say only imprecisely — they can be normal in both. More important than the single value is therefore the question of scarring, and that is now estimated with elastography or a score calculated from several blood values, no longer routinely with a tissue sample. The encouraging part: the liver forgives a lot. Seven to ten per cent weight loss brings most of the fat back out, and even five per cent shows an effect.
It rarely comes alone: waist, blood pressure, sugar and blood fats belong looked at with it. Seeing all five together also shows why all five move together.
In fatty liver a raised ferritin is common and usually harmless. If it stays high and the transferrin saturation is raised as well, that is another story.
The most effective lever; de-fats the liver and can push back inflammation.
Sweet drinks and fruit juices go into the liver as fructose. Leaving them out is the step with the best ratio of effort to effect.
Not whether fat is there but whether scarring is already there decides the course. You can ask for this explicitly — it is not always offered.
The name only says alcohol is not the cause. It still adds to it, and weighs more heavily on a liver already affected.
Muscles take up sugar and relieve the liver. Cutting only the amount loses muscle too — and with it the part that helps.
For the liver, regular coffee is linked in observational studies with less scarring. No reason to start, but none to stop either.
About one adult in four has a fatty liver, considerably more with excess weight. In the early stages it is fully reversible: a weight loss of seven to ten percent makes the fat measurably disappear — one of the few diagnoses with a real reverse gear.
For fatty liver there is no drug that takes the fat out of the liver. What works measurably is weight: seven to ten per cent less body weight lets inflammation and partly scarring recede in a large share of people — seen over six to twelve months. SGLT2 inhibitors and GLP-1 drugs lower liver fat because they lower weight. Vitamin E lowered inflammation markers in non-diabetics in studies; the question of duration is open.
A fatty liver does not hurt. It is nearly always found in passing — on an ultrasound done for something else, or through a slightly raised liver value.
A dull pulling under the right ribs when the liver enlarges. Non-specific and often explained otherwise.
Often mentioned, hard to attribute — it belongs just as much to everything that accompanies a fatty liver.
Waist measurement says more here than the scales. A slim person with insulin resistance can have a fatty liver.
Normal values rule out neither the fatty liver nor the scarring. So the question of scarring matters more than the single value.
Weight & sugar reduction help most.
The only measure that demonstrably reverses the fatty liver — even before any weight loss.
The traditional liver remedy par excellence; the studies in fatty liver are mixed.
Fructose from drinks and processed food goes straight to the liver — the biggest lever is here.
