Optic atrophy

Atrophia nervi optici

DE FR EN

A pale optic nerve is a finding, not a diagnosis — the cause decides what there is left to protect.

Also known as: optic atrophy · loss of optic nerve fibers

Optic atrophy means the optic nerve has lost fibres. In the back of the eye this is seen in a pale optic disc. That is a finding and not yet a diagnosis — the real question is always: what damaged the nerve? Quite different roads lead here. Too high a pressure in the eye over years, a circulation problem, an inflammation of the optic nerve, a shortage or a poison, pressure from outside by a mass, an injury — and congenital or inherited forms that begin in childhood or in young adulthood. Lost fibres do not grow back. So it is never about getting back what is gone, but about two things: protecting the rest and preserving the second eye. That is why the search for the cause matters and is not a formality. Two roads are especially urgent: a new loss of sight in someone over fifty with headache or pain on chewing, because that can be an inflammation of the arteries, and a one-sided finding without a clear explanation, which calls for imaging of the head.

Levers & pathways

studiedHave the cause named

After the diagnosis the central question is: by what? While it stays open, nobody knows whether anything can be switched off — or whether the second eye is at risk.

studiedWith new worsening, ESR and CRP at once

Over fifty, with headache, pain on chewing or shoulder pain: this combination does not tolerate an appointment next week.

studiedKeep an eye on the pressure in the eye

Pressure is the one cause that can be treated directly over years. Even if it was not the trigger, a low pressure protects the fibres that remain.

studiedHave the shortage clarified

A B12 shortage is one of the few correctable causes and is often overlooked, because it comes slowly and at first only makes colours paler.

studiedHave the visual field measured regularly

It is not the sharpness but the visual field that shows whether anything is progressing. What matters is keeping the same examination so the values stay comparable.

studiedProtect the second eye

With circulation problems of the optic nerve, blood pressure, blood sugar, blood fats, smoking and night-time breathing all count. Anyone who snores at night and is tired by day should be looked at for that.

associatedThe causes that can be switched off

Before the optic nerve turns pale there is a window: vitamin B12 shortage, alcohol, smoking and individual medicines are causes that can be taken away — and then part of the sight often recovers.

infoWhat the medicine does — and what it does not

An atrophied optic nerve does not grow back — no treatment restores it, and that is why everything hangs on the cause and the timing. What can be treated is treated, so that it does not go further: lowering pressure in the eye, stopping inflammation, refilling B12, removing the trigger. Idebenone has been studied in Leber’s hereditary form. What counts afterwards is adaptation: magnifying aids, contrast, lighting, orientation training.

infoNumbers and timeframes

The paleness of the disc becomes visible four to six weeks after the damage — before that the nerve looks normal although it is already damaged. So a normal-looking fundus in the acute phase says nothing. The optic nerve has about 1.2 million fibres; a measurable field loss arises once a considerable share of them is lost. OCT measures the nerve fibre layer in micrometres and shows the loss earlier than the naked eye.

Possible complaints

associatedThe optic disc is pale

This finding is made by the eye doctor when looking into the back of the eye. It says that substance has been lost — but nothing about what caused it. That is exactly why a search for the cause always follows.

associatedThe visual field reveals the road

A loss in the middle speaks for inflammation, shortage or poison; a loss at the edge and arc-shaped more for pressure; a loss that stops exactly at the vertical midline directs the view behind the eye.

associatedOne eye only, with no explanation

A one-sided finding without a clear reason calls for imaging of the head, because pressure on the nerve from outside can develop slowly and painlessly.

associatedIt has always been like this

Congenital and inherited forms are often found by chance or begin at a young age, sometimes in several family members. Here little usually changes, and support counts more than searching.

associatedNew, over fifty, with headache

New headaches, pain on chewing, a tender temple and a loss of sight are together an emergency. Treatment starts on suspicion, not after proof — the second eye is at stake.

Blood values

studiedESR (sedimentation rate)

With new visual loss after the age of fifty the most urgent value of all, because together with the CRP it points to an inflammation of the arteries — which is treated before the finding is confirmed.

studiedCRP

Always belongs measured together with the sedimentation rate. The two together detect the inflammation of the arteries more reliably than either alone.

studiedVitamin B12

One of the few roads that can be switched off. So this value belongs alongside even when another cause seems obvious.

associatedFasting glucose

A blood sugar raised over many years damages the small vessels that supply the optic disc. It does not explain the finding on its own but belongs to the picture.

Nutrients

studiedVitamin B12

Relevant if deficiency-related.

Supplements

studiedVitamin B12

The most important reversible cause in this list, and it is recognised late because it comes creeping: colours fade first, then the middle of the picture. With nerve damage B12 is injected and not swallowed — and the longer the gap lasted, the less comes back.

studiedB complex (folate, B1)

With heavy, long alcohol use several gaps come together — B1, folate and B12 — and the optic nerve is one of the organs that show it. Here replacement works if it comes early enough; that is one of the honest good pieces of news in this field.

no studiesCaution with “optic nerve cures”

For an optic nerve that has already wasted much is offered, and none of it has a demonstrated effect. What actually counts is the search for the cause while the other side is still healthy — the lost fibre does not come back, the second one can be protected.

Hormones

studiedProlactin

A pituitary tumour presses on the crossing of the optic nerves from below — the loss then affects the outer visual field on both sides. Prolactin is the value that shows the most common of these growths in a single blood draw.

studiedT4 (thyroxine)

With the eye involvement of Graves’ disease the swollen tissue behind the eye can pinch the optic nerve. That is an emergency and the reason double vision and fading colour vision are taken seriously there.

Alternative approaches — what people use

studiedVitamin B12

If a nutrient deficiency is the cause, progression can be halted — fibres already lost do not return. That is why measurement comes early with unexplained optic nerve loss.
