A disruption of speech fluency: sounds, syllables or words are repeated, prolonged or blocked. It usually begins in childhood (around 2–5 years) and often resolves on its own. Stuttering has a neurobiological and hereditary basis — it is not nervousness, not a sign of anxiety and not a question of upbringing. Excitement can intensify it but does not cause it. Speech therapy helps in living with it more easily. How this is worked on in speech therapy — amount, duration, limits and the signs that are urgent — is ordered by age at logopaedie.borters.ch.

Levers & pathways

studiedSpeech therapy

The most important help. Teaches techniques for smoother, more relaxed speech; for children, early and playful.

studiedTake off the pressure, listen patiently

Let them finish, don’t interrupt or complete sentences. This doesn’t worsen it — but it relieves and reduces shame.

studiedAssess early (in children)

If it lasts longer than about six months or the child suffers, get a speech-therapy check — chances are best early.

associatedUnderstanding & self-help

Knowing that stuttering is neurobiological (no one’s fault) takes off pressure. Self-help groups help many.

infoWhat the medicine does — and what it does not

There is no drug for stuttering. What works is therapy: in children the Lidcombe programme with parental feedback, in older children and adults approaches that reshape speech or change the stuttering itself. In about three in four children who begin to stutter between two and five, it stops by itself within one to two years. Early therapy is nonetheless effective — waiting until school entry is no longer regarded as the better route.

infoNumbers and timeframes

About five in a hundred children stutter at times, usually beginning between two and five; in about one in a hundred adults it stays. In about three in four children it stops by itself within one to two years. Boys are affected about three times as often — and among the children in whom it stays, the ratio is even clearer. The Lidcombe programme works with about twelve to sixteen weekly appointments and short daily practice at home.

studiedWhy it often falls silent in singing

Stuttering fluctuates with the situation — stronger on the phone and before groups, often gone in singing. That shows how much the control of speech depends on the setting.

Possible complaints

studiedRepetitions, prolongations, blocks

The three core forms of stuttering in speech.

studiedAssociated movements & speech effort

Facial or body tension, visible struggle for the word.

associatedSpeech fear & avoidance

Some avoid certain words or situations — often more burdensome than the stuttering itself.

Nutrients

studiedVitamin B12

A lack causes exhaustion, problems concentrating, low mood and forgetfulness — and it is one of the few causes that can be found and put right with a single blood draw.

associatedVitamin D

Low values are common with low mood. Whether correcting it lifts the mood is not settled; that it makes sense in winter at our latitudes is.

studiedIron

An empty store causes tiredness, lack of drive and problems concentrating long before any anaemia becomes visible. In women of childbearing age the most common single reason.

associatedOmega-3 fatty acids

A building block of the cell coverings in the brain. The studies on mood are inconsistent; as part of a good diet it belongs there anyway.

associatedFolate

Needed for making the messengers in the brain. It runs short with alcohol and under certain medicines — and it is always judged together with B12.

Supplements

studiedWhy little stands here

Honestly: no preparation improves fluency. What works is speech therapy, and in children the timing matters more than any ingredient. What counts here is sleep — under tiredness the stammer visibly increases in most people.

mixedMagnesium

Often recommended because tension worsens fluency. On the stammer itself no effect is shown; if it helps via better sleep, that is the route — and then one says so.

infoVitamin B12

Clearly worthwhile and well supported when deficient — vegan, older adults, certain drugs. No energy boost without a deficiency.

infoVitamin D3 (+K2)

Clearly worthwhile and well studied when deficient; fewer data for the wider uses. Fat-soluble — check blood levels at high doses.

infoIron

Clearly effective with proven deficiency, useless without. Take only with a documented deficiency — excess is harmful.

infoOmega-3 (EPA/DHA)

Effects on inflammation and triglycerides established; cardiovascular prevention is mixed. Watch quality and freshness.

infoFolic acid / folate

In pregnancy clearly established against neural-tube defects, otherwise mainly with deficiency. Can mask a B12 deficiency — watch both.

Hormones

mixedDopamine (PIF)

Imaging studies show raised dopamine activity in certain areas with stuttering. Medicine trials were derived from that and all failed on side effects — speech therapy remains the route that works.

associatedTestosterone

Boys are affected about four times as often as girls, and in girls it more often disappears by itself. Why is open — the difference is large enough to be named.

Food

associatedCaffeine in children and adolescents

Caffeine increases tension, and tension increases stuttering. Energy drinks hold 80 mg and more per can — with teenagers this is worth a look.

Alternative approaches — what people use

mixedBreathing exercises

Breathing techniques are part of speech therapy programmes but do not work on their own. What helps most is the environment: let them finish, do not complete their sentences, do not push for slower speech.
