Zinc deficiency

Deficientia zinci

DE FR EN

The zinc value in blood falls with any inflammation — it is measured fasting and read together with CRP and albumin, otherwise it says little.

Zinc sits in hundreds of enzymes and is needed wherever something is newly made: in the skin, in wound healing, in defence, in the sense of taste. There is no store, so supply depends on the daily intake. The blood value, though, is a difficult witness and is more often misread than mismeasured: it falls with any inflammation, with a low albumin, after eating and across the day. A low value with a raised CRP therefore proves no shortage, and a normal one does not reliably rule it out. It is measured fasting in the morning and read together with CRP and albumin. Common reasons for a real shortfall are a mainly plant-based diet, because phytates from grains and pulses bind zinc, along with alcohol, chronic diarrhoea, coeliac and Crohn disease, surgery on the stomach, water tablets and high-dose iron or calcium supplements. What shows itself is a loss of taste and smell, wounds that heal badly, reddened skin around mouth and nose, diffuse hair loss and frequent infections. In correcting it the duration counts: weeks make sense, months at a high dose crowd out copper.

Levers & pathways

studiedMeasure fasting and read the CRP with it

Without the inflammation value beside it a low zinc cannot be read. That saves months with a supplement that was never missing.

studiedSoak, sprout, sourdough

Soak pulses overnight and pour the water away, bread with sourdough rather than yeast. These three moves clearly raise uptake without changing the menu.

studiedSpace zinc away from iron and calcium

High-dose iron and calcium hinder zinc uptake by the same route. Anyone taking both leaves several hours between them.

studiedNot months at a high dose

Weeks make sense, long-term high dosing crowds out copper — with anaemia, few white cells and nerve damage as the result. Anyone taking it longer has the copper checked too.

studiedWith loss of taste check the other reasons

Zinc is one of several explanations. Medicines, infections, dental problems and the nose itself are commoner and belong looked at first.

studiedOysters, meat, cheese, seeds, oats

From animal sources zinc is taken up considerably better. In a plant diet, pumpkin and hemp seeds, cashews, lentils and oats are the most generous items.

infoWhat the medicine does — and what it does not

Zinc works in a cold only as a lozenge started within twenty-four hours of the first signs and sucked every two to three hours — it acts locally in the throat, not through the blood, and shortens the course by one to two days. With a documented deficiency, wound healing, skin and taste improve over four to twelve weeks. Above 40 milligrams a day over months, zinc lowers copper uptake and can cause anaemia.

infoNumbers and timeframes

The daily requirement is about 7 to 16 milligrams, depending on the phytate content of the food — wholegrains and pulses bind zinc, soaking and sourdough release some of it again. The blood value is unreliable: it falls with any inflammation, regardless of the store. In a cold, zinc works only as a lozenge and only if started within twenty-four hours. Above 40 milligrams a day over months it blocks copper uptake.

Possible complaints

associatedFood tastes of nothing any more

Taste and smell are among the first senses to suffer. That in turn reduces appetite and so closes a circle.

associatedWounds take too long

Cuts, sore places and small injuries heal sluggishly. After operations and with chronic wounds the value is therefore particularly worthwhile.

associatedReddened, scaly skin around mouth and nose

Also on hands, elbows and, in a child, in the nappy area. This distribution is one of the few genuinely typical pictures.

associatedA mainly plant-based diet

The problem is not the amount but the availability: phytates from wholegrain and pulses bind zinc in the gut. Soaking, sprouting and sourdough solve a good part of it.

associatedA gut that does not hold on

Chronic diarrhoea, coeliac disease, Crohn and stomach surgery are the commonest medical reasons. Alcohol also raises the loss through the kidney.

studiedSigns on the body

What a deficiency makes visible on the body — skin, nails, tongue, eyes and nerves. The signs are unspecific: they show where to look, they prove nothing.

Blood values

studiedZinc in serum

Draw fasting in the morning and not during an infection. The draw itself counts too: zinc from rubber stoppers can falsely lift the value.

studiedCRP

The value without which the zinc cannot be judged. With a raised CRP zinc moves out of the blood into the liver — that looks like a shortage and is not one.

studiedAlbumin

Most of the zinc in blood is bound to albumin. If the albumin is low, the zinc is low too without anything being missing.

studiedAlkaline phosphatase

A zinc-dependent enzyme. A strikingly low alkaline phosphatase is a rarely used but usable side hint of a genuine zinc shortage.

Nutrients

studiedZinc

The missing building block.

studiedVitamin A

Works with zinc for skin/immunity.

Supplements

studiedZinc

In courses; high doses lower copper.

Hormones

studiedTestosterone

With a proven zinc deficiency testosterone rises again with correction. Without a deficiency nothing moves — that is the whole difference between replacing and boosting, and it is rarely said this plainly.

associatedT3 (triiodothyronine)

Zinc is needed to convert T4 into active T3 and for the hormone to bind to its receptor. A deficiency can therefore cause the complaints of an underactive thyroid although the gland itself is healthy.

associatedIGF-1

In children with pronounced zinc deficiency growth falls behind, and correcting it catches part of that up. In Europe this is rare — worldwide it is one of the most common reasons for delayed growth.

Food

studiedMeat, cheese, pumpkin seeds, oats, legumes

Animal zinc better absorbed.

Alternative approaches — what people use

studiedZinc

Better absorbed on an empty stomach but less well tolerated — and taken separately from iron.

studiedDiet change

Phytates from grains and pulses slow the uptake; soaking and sourdough reduce them.
