Vitiligo
Pigment cells are lost — harmless physically, but often burdensome emotionally.
Also known as: vitiligo · white-patch disease

An autoimmune condition in which the skin’s pigment-making cells are lost. Sharply defined white patches appear, often symmetric on hands, face or around body openings. Vitiligo is not contagious, not painful and not dangerous to physical health — but it can be emotionally hard. It occurs more often together with other autoimmune conditions (e.g. thyroid). The course varies widely. Treatments can bring pigment back or even out the appearance; the pale areas need good sun protection. Acceptance and support matter just as much.
In vitiligo the pigment is missing completely, the areas are chalk-white, sharply bordered and do not scale. If they scale finely, it is something else — and quickly treatable.
Consistent protection prevents sunburn and reduces the contrast.
Creams (e.g. anti-inflammatory agents) and phototherapy can encourage pigment — the earlier, the better the odds.
As the thyroid is often involved, checking thyroid values is worthwhile.
Exchange and, if wished, camouflage techniques help day to day. Vitiligo changes appearance, not a person’s worth.
Cortisone creams and calcineurin inhibitors slow the attack on the pigment cells — colour returns, if at all, out of the hair roots, which is why the face does best and hands and feet worst. Visible at the earliest after three months, judged at six. Narrowband UVB two to three times a week over six to twelve months is the route with the most data. Ruxolitinib cream is newer and works mainly on the face.
About one in a hundred people is affected, and it usually begins before the age of thirty. In about one in five a thyroid condition is also found — which is why the value is checked once. A treatment attempt is first judged at three months and decided at six. Colour returns out of the hair roots: on the face in about half of people, on hands and feet far less often, because there are fewer hair roots there.
Pigment-free areas, often symmetric and slowly growing.
The pigment-free spots burn especially easily in the sun.
Visible patches can weigh on self-image — although physically harmless.
The most important accompanying value. A thyroid illness occurs distinctly more often with this skin picture and is often only found through it.
Show the predisposition even when the TSH is still normal. They change nothing about the skin, but they place the course.
Belongs alongside, because the same kind of misdirection can also affect the uptake of B12 in the stomach.
With chronic inflammatory and autoimmune pictures low levels are common — partly because the illness itself lowers the level, partly because less time is spent outdoors. Whether correcting it changes the course is being studied; for bone and muscle it is worthwhile regardless.
Skin and lining renew themselves quickly and need zinc for it. With places that heal badly, cracked corners of the mouth and frequent skin infections a lack is a serious possibility.
They give rise to messengers that tend to end inflammatory reactions rather than fuel them. Oily sea fish twice a week is the simplest route; capsules are an option but replace no treatment.
Without vitamin C no stable collagen forms — the wound closes but holds less well. With a very one-sided diet, smoking and alcohol the intake is often tighter than assumed.
Steers the horn-forming of the skin. Too little makes it rough and dry, too much over longer periods harms liver and bone — and in pregnancy a strict upper limit applies.
One of the few swallowed remedies with a controlled study behind it: 60 mg twice daily over 6 months showed the spread halting and partial repigmentation in slowly progressing vitiligo. The groups were small. It influences clotting.
Low levels are more common with vitiligo, as with other autoimmune situations. Measured and replaced if deficient — repigmentation from vitamin D alone is not shown.
Vitiligo occurs more often together with pernicious anaemia and thyroid disease. That is the reason to measure B12 and the thyroid values once — not as a treatment for the patches.
Effects on inflammation and triglycerides established; cardiovascular prevention is mixed. Watch quality and freshness.
About one in five people with vitiligo also has an autoimmune thyroid condition. That is the reason to measure TSH once — not because of the patches, but because of what lies alongside.
This hormone steers the pigment cells. With vitiligo the cells themselves are missing — the signal arrives, but there is no longer anyone to answer it.
Vitiligo occurs more often together with adrenal insufficiency. Skin darkening at scars and hand creases alongside the white patches is the combination that then does not belong ignored.
In vitiligo these three are more often low than average, often alongside thyroid disease. Correction does not bring the colour back but belongs to the work-up.
In vitiligo the level is more often low, often together with thyroid disease. Correction does not restore the colour — light therapy is the route that works.
Two small studies found with 40–60 mg three times daily over 6 months a halt in spreading and partial repigmentation. The studies are small and have not been confirmed.
